摘 要:临床危重症患者多处于高分解代谢状态,易发生营养不良,营养不良是影响其临床结局的重要风险因素.营养支持作为重症监护病房(ICU)中器官功能支持治疗的重要手段,在危重症患者预后中发挥着重要的作用.及时、准确评估患者的营养风险是营养支持和有效改善预后的前提.目前在临床中有多种简单、可靠、快速的评估方法用于评估患者的营养状况,危重症营养风险(NUTRIC)评分和改良NUTRIC(mNUTRIC)评分是目前危重症患者营养状况的特异性评价工具,其不仅为精细化营养支持策略提供了科学依据,有利于患者营养状况的改善,进而减少营养不良事件的发生;而且还可预测患者疗效与预后结局,为临床治疗提供参考.现就NUTRIC与mNUTRIC评分系统在危重症患者中应用的研究进展进行综述,以进一步推广营养风险筛查的标准应用,推进营养支持尤其早期营养支持的开展,提高危重症患者营养康复科学管理水平.Critically ill patients are mostly in a state of high catabolism and are more prone to malnutrition.Malnutrition is an important risk factor affecting their clinical outcomes.As a vital measure of organ function support treatment in the intensive care unit(ICU),nutritional support plays a crucial role in the prognosis of critically ill patients.Timely and accurate assessment of nutritional risk is a prerequisite for nutritional support and effective improvement of prognosis.At present,there are a variety of simple,reliable,and rapid assessment methods to assess the nutritional status of patients.Nutrition risk in the critically ill(NUTRIC)and modified NUTRIC(mNUTRIC)scores are specific assessment tools for nutritional status in critically ill patients.They not only provide scientific basis for fine nutritional support strategies,which are helpful to improve nutritional levels and decrease malnutrition for patients but also predict curative effects and prognosis,which can provide practical reference for clinical treatment.This paper reviews the application of NUTRIC and mNUTRIC scoring systems in critically ill patients,aiming to further spread standard application of nutritional risk screening and advance nutrition support especially early nutrition support,and improve science management of nutrition rehabilitation for critically ill patients.
