达芬奇机器人系统与传统腹腔镜直肠癌根治术治疗直肠癌临床疗效比较

摘　　要：目的比较达芬奇机器人系统与传统腹腔直肠癌根治术治疗直肠癌的临床疗效。方法选取北部战区总医院普通外科自2017年1月至2019年12月收治的386例行达芬奇机器人系统与传统腹腔镜手术患者为研究对象。根据不同的手术方式将患者分为A组(n=268)与B组(n=118)。A组患者采用传统腹腔镜直肠癌根治术治疗,B组患者采用达芬奇机器人系统直肠癌根治术治疗。记录并比较两组患者的基线资料、围术期指标、并发症发生情况、存活率。结果B组患者手术时间长于A组,术中出血量少于A组,差异均有统计学意义(P<0.05)。A组、B组患者并发症发生率分别为8.2%(22/268)、7.6%(9/118),差异无统计学意义(P>0.05)。A组、B组患者1、3年总存活率与无病存活率比较,差异均无统计学意义(P>0.05)。结论与传统腹腔镜直肠癌根治术比较,达芬奇机器人系统直肠癌根治术治疗直肠癌的手术时间更长,但出血量更少,两种手术方式均安全有效。Objective To compare the clinical efficacy of Da Vinci robot system and traditional laparoscopic radical resection of rectal cancer in the treatment of rectal cancer.Methods A total of 386 patients with Da Vinci robot system and traditional laparoscopic surgery admitted to the Department of General Surgery of General Hospital of Northern Theater Command from January 2017 to December 2019 were selected as the study objects.Patients were divided into group A(n=268)and group B(n=118)according to different surgical methods.Group A was treated with traditional laparoscopic radical resection of rectal cancer,group B was treated with Da Vinci robotic system radical resection of rectal cancer.The baseline data,perioperative indexes,complications and survival rate of the two groups were recorded and compared.Results The operative time of group B was longer than that of group A,and the intraoperative blood loss was less than that of group A,the differences were statistically significant(P<0.05).The incidence of complications in group A and group B was 8.2%(22/268)and 7.6%(9/118),respectively,with no statistical significance(P>0.05).There was no significant difference in 1-year and 3-year overall survival rate and disease-free survival rate between group A and group B(P>O.05).Conclusion Compared with the traditional laparoscopic radical resection of rectal cancer,the Da Vinci robotic system radical resection of rectal cancer has a longer operation time,but less blood loss.Both surgical methods are safe and effective.
