2型糖尿病牙周炎患者龈沟液瘦素水平与牙周临床指标的相关性研究

摘　　要：目的:研究2型糖尿病牙周炎患者龈沟液中瘦素(LEP)水平及其与牙周临床指数的关系。方法:随机选择健康体检者(A组)、慢性牙周炎患者(B组)和2型糖尿病牙周炎患者(C组),每组30例,记录所有受检者的牙周袋探诊深度(PD)、牙周附着水平(AL)、菌斑指数(PLI)及探诊出血(BOP)。用高效液相层析法检测各组受检者糖化血红蛋白(HbA1c),ELISA法检测龈沟液中LEP水平。结果:C组血清中HbA1c浓度均显著高于B组、A组(P<0.05);C组龈沟液LEP浓度均显著高于B组、A组(P<0.05);B组与A组相比,HbA1c水平无明显的差别(P>0.05),龈沟液中LEP浓度较高(P<0.05)。在所有受试者中,龈沟液瘦素含量指标值与PD、AL、PLI、BOP指标值均呈正相关,r值分别为0.884、0.883、0.881和0.772(P<0.05)。结论:2型糖尿病伴牙周炎患者,龈沟液中瘦素水平显著升高;龈沟液LEP水平与PD、AL、PLI、BOP指标呈正相关。Objective:To detect the association of leptin(LEP)level in gingival crevicular fluid(GCF)with clinical periodontal indexes in patients of type 2 diabetes mellitus and periodontitis.Methods:30 healthy subjects(group A),30 chronic periodontitis patients(group B)and 30 type 2 diabetic patients with chronic periodontitis(DMCP)(group C)were included.Probing depth(PD),attachment loss(AL),plaque index(PLI)and bleeding on probe(BOP)were measured.High performance liquid chromatography detection was used to determine the expression of the Glycated hemoglobin(HbA1c)in plasma samples.LEP level in GCF was measured by ELISA.Results:The level of serum HbA1c in group C was higher than that in group B and A(P<0.05);GCF LEP in group C was higher than that in group B and A(P<0.05).GCF LEP in group B was higher than that in group A(P<0.05).However,no virtual disparity was found in serum HbA1c between group A and B(P>0.05).The GCF LEP level was positively correlated with PD,AL,PLI and BOP,and the r values were 0.884,0.883,0.881 and 0.772 respectively(P<0.05).Conclusion:The level of LEP is elevated in DMCP patients.GCF LEP level is positively correlated with periodontal parameters of PD,AL,PLI and BOP.
