摘 要:目的:采用三维测量方法分析下颌前突患者双侧下颌骨升支矢状劈开术(BSSRO)后颏唇区轮廓变化。方法:纳入16名下颌前突患者,手术方式为BSSRO或BSSRO+Le Fort I型截骨术。收集术前1周(T0)、术后6~8个月(T1)螺旋CT数据,使用mimics软件建模并导入3-matic软件拟合、测量颏唇区软硬组织标志点三维移动距离及下唇区、颏部和颏唇区体积变化。并对颏唇区软、硬组织标志点移动距离、各分区体积变化和平均移动距离行Pearson相关性和线性回归分析。结果:颏唇区软硬组织标志点在三维方向变化均有显著相关性(P<0.01)。下唇区及颏部软硬组织体积变化以及平均移动距离均有显著相关性(P<0.05)。结论:下颌前突患者BSSRO术后颏唇区软硬组织变化在三维方向均有显著相关性,且能够准确预测。Objective:To study the changes of mentolabial profile in patients with mandibular protrusion after orthognathic surgery using 3D measurement,and to analyze the influence factors.Methods:16 patients with mandibular protrusion underwent only bilateral sagittal split ramus osteotomy(BSSRO)or bimaxillary surgery were included.CT data at 1 week preoperative(T0)and 6-12 months after surgery(T1)were collected respectively.The 3D model was constructed by Mimics,and the 3D changes in soft and hard tissue landmarks and regions of lower lip,chin and mentolabial area were evaluated by 3-Matic.Pearson correlation and linear regression models were performed in this study.Results:All landmarks showed significant correlation in 3D direction(P<0.01).The region-based study demonstrated that there were significant correlations between the soft tissue and hard tissue in lower lip region,chin regionin volumetric changes and average movement(P<0.05).Conclusion:BSSRO can prduce significant correlation between soft tissue and hard tissue landmarks in 3D changes in mentolabial region,and the changes can be accurately predicted.
