摘　　要：目的:分析腺样体切除术联合耳内镜下鼓膜置管治疗儿童分泌性中耳炎的效果。方法:选取2020年1月—2023年1月黔东南苗族侗族自治州人民医院收治的分泌性中耳炎患儿94例。采用随机数字表法分为参比组与应用组各47例。参比组采用耳内镜下鼓膜置管术治疗;应用组采用腺样体切除术联合耳内镜下鼓膜置管治疗。对比两种手术方法疗效指标、炎症反应、听力水平、手术安全性、术后复发率。结果:应用组积液消失时间、鼓膜痊愈时间、耳闷塞感消失时间、听力恢复时间均早于参比组,差异有统计学意义(P<0.05)。治疗后,应用组听力水平优于参比组,差异有统计学意义(P<0.05)。应用组总并发症发生率、复发率均低于参比组,差异有统计学意义(P<0.05)。结论:腺样体切除术联合耳内镜下鼓膜置管治疗儿童分泌性中耳炎能够减轻机体炎症反应,加快症状消失,尽早恢复听力,且安全性高,复发率低。Objective To evaluate the effect of adenoidectomy combined with eardrum catheterization under otoendoscope in the treatment of secretory otitis media in children.Methods 94 children with secretory otitis media admitted to the People’s Hospital of Qiandongnan Miao and Dong Autonomous Prefecture from January 2020 to January 2023 were selected as subjects.Random number table method was used to divide the patients into two groups with 47 cases in each group.The reference group was treated with tympanic membrane catheterization under otoendoscope.The application group was treated with adenoidectomy and tympanic membrane catheterization under otoendoscope.The observation indexes,inflammatory response,hearing level,surgical safety and postoperative recurrence rate were compared between the two surgical methods.Results The disappearance time of effusion,healing time of tympanic membrane,disappearance time of ear congestion and hearing recovery time in application group were earlier than those in reference group,and the differences were statistically significant(P<0.05).After treatment,the hearing level of the application group was better than that of the reference group,and the difference was statistically significant(P<0.05).The complication rate and recurrence rate of the application group were lower than those of the reference group,and the difference was statistically significant(P<0.05).Conclusion Adenoidectomy plus tympanic membrane catheterization under otoendoscope in the treatment of secretory otitis media in children can reduce the inflammatory response of the body,accelerate the disappearance of symptoms,restore hearing as soon as possible,with high safety and reduce recurrence.
