腹腔镜辅助小切口胃癌根治术治疗远端胃癌的疗效

摘　　要：目的:探讨腹腔镜辅助小切口胃癌根治术治疗远端胃癌的效果。方法:选取2018年1月—2019年12月柳州市工人医院收治的远端胃癌患者88例为研究对象,根据随机数字表法分为对照组和观察组,各44例。对照组采用传统开腹手术治疗,观察组采用腹腔镜辅助小切口胃癌根治术治疗。比较两组恢复情况、血清肿瘤标志物指标、术后并发症发生率。结果:观察组住院时间、胃肠道功能恢复时间、术后开始进食时间短于对照组,差异有统计学意义(P=0.001)。术后1个月、2个月、3个月观察组癌胚抗原、甲胎蛋白、糖类抗原125、鳞状细胞癌相关抗原水平低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P=0.044)。结论:腹腔镜辅助小切口胃癌根治术对远端胃癌患者的临床治疗效果显著,能够降低术后血清学肿瘤标志物水平及并发症发生率。Objective:To investigate the effect of laparoscopy-assisted small incision radical gastrectomy for distal gastric cancer.Methods:A total of 88 patients with distal gastric cancer who were admitted to Liuzhou Worker's Hospital from January 2018 to December 2019 were selected as the research subjects,and they were divided into the control group and the observation group according to the random number table method,with 44 cases in each group.The control group was treated with traditional laparotomy,and the observation group was treated with laparoscopy-assisted small incision radical gastrectomy.The recovery,serum tumor markers and incidence of postoperative complications were compared between the two groups.Results:The hospitalization time,gastrointestinal function recovery time,and postoperative eating time in the observation group were shorter than those in the control group,and the difference was statistically significant(P=0.001).The levels of carcinoembryonic antigen,alpha-fetoprotein,carbohydrate antigen 125,and squamous cell carcinoma-associated antigen in the observation group at 1 month,2 months,and 3 months after operation were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.044).Conclusion:Laparoscopic assisted small incision radical gastrectomy has a significant clinical effect on patients with distal gastric cancer,and can reduce the levels of serum tumor markers and the incidence of complications after surgery.
