摘 要:目的 分析血清可溶性MHCⅠ类链相关分子A(sMICA)、microRNA-99a、DJ-1蛋白(DJ-1)对非小细胞肺癌(NSCLC)诊断及预后评估价值。方法 收集2019年6月至2022年6月本院收治的NSCLC患者90例(NSCLC组),另选取本院同期健康体检志愿者90例(对照组)。对患者进行为期6个月随访,随访截止至2023年1月。采用ROC曲线评估sMICA、microRNA-99a、DJ-1对NSCLC诊断价值。采用多元Logistic回归分析影响NSCLC患者预后的关系。结果 NSCLC组sMIC、DJ-1表达水平高于对照组,microRNA-99a表达水平低于对照组(P<0.05);ROC曲线结果显示:sMICA、microRNA-99a、DJ-1对NSCLC诊断AUC值分别为0.742(95%CI:0.642~0.842)、0.759(95%CI:0.660~0.859)、0.789(95%CI:0.698~0.901)。90例患者中预后良好69例,预后不良21例。预后不良者sMIC、DJ-1表达水平高于预后良好组,microRNA-99a表达水平低于预后良好组(P<0.05);预后不良者TNM分期(III-IV期)、低分化占比高于预后良好组(P<0.05);两组在年龄、性别、肿瘤直径、病理类型中比较差异无统计学意义(P>0.05)。经多元Logistic回归分析:临床分期、分化程度、sMICA、microRNA-99a、DJ-1为影响NSCLC患者预后不良的危险因素(P<0.05)。结论 相对健康人群,NSCLC患者sMICA、DJ-1水平高,microRNA-99a水平低;三指标水平与患者预后密切相关,通过检测血清三者水平可为患者后续诊疗、预后评估提供参考资料。Objective To evaluate the value of serum soluble MHC ClassⅠchain related molecule A(sMICA),microRNA-99a and DJ-1 protein(DJ-1)in the diagnosis and prognosis of non-small cell lung cancer(NSCLC).Methods Ninety patients with NSCLC admitted to our hospital from June 2019 to June 2022(NSCLC group)were collected,and 90 healthy volunteers(control group)were selected during the same period.Patients were followed up for 6 months until January 2023.ROC curve was used to evaluate the diagnostic value of sMICA,microRNA-99a and DJ-1 in NSCLC.Multiple Logistic regression was used to analyze the relationship between NSCLC patients and prognosis.Results The expression levels of sMIC and DJ-1 in NSCLC group were higher than those in control group,and the expression levels of microRNA-99a in NSCLC group were lower than those in control group(P<0.05).ROC curve results showed that the AUC values of sMICA,microRNA-99a and DJ-1 for the diagnosis of NSCLC were 0.742(95%CI:0.642-0.842),0.759(95%CI:0.660-0.859)and 0.789(95%CI:0.698-0.901),respectively.Of the 90 patients,69 had good prognosis and 21 had poor prognosis.The expression levels of sMIC and DJ-1 in poor prognosis group were higher than those in good prognosis group,and the expression levels of microRNA-99a in poor prognosis group were lower than those in good prognosis group(P<0.05).The proportion of TNM stage(III-IV)and low differentiation in poor prognosis group was higher than that in good prognosis group(P<0.05).There was no significant difference in age,sex,tumor diameter and pathological type between the two groups(P>0.05).Multiple Logistic regression analysis showed that clinical stage,differentiation degree,sMICA,microRNA-99a and DJ-1 were the risk factors for poor prognosis of NSCLC patients(P<0.05).Conclusion Compared with the healthy population,the level of sMICA and DJ-1 in NSCLC patients is higher,and the level of microRNA-99a is lower.The level of three indicators is closely related to the prognosis of patients,and the detection of serum levels of three indicat
