摘  要:目的 探讨磁共振动态增强扫描(DCE-MRI)定量参数在骨肿瘤良恶性鉴别诊断中的价值。方法 回顾性分析经手术或穿刺病理诊断为骨肿瘤且已完成DCE-MRI检查的患者102例,分为良性(29例)和恶性骨肿瘤组(73例)。分别计算两组患者的定量参数Ktrans和Kep等,并进行独立样本t检验,应用受试者工作特征曲线评估各指标的诊断效能。结果 Ktrans值和Kep值在良恶性骨肿瘤间差异有统计学意义(F=10.578和18.154,P<0.05)。当Ktrans值和Kep值的诊断切点值分别为0.822(min^(-1))和2.292(min^(-1))时,诊断恶性骨肿瘤的灵敏度分别为89.04%,65.75%,特异度分别为82.76%,89.66%。结论 DCE-MRI定量参数有助于鉴别良恶性骨肿瘤。Objective To investigate the value of quantitative parameters of dynamic contrast-enhanced MRI(DCE-MRI)in differential diagnosis of benign and malignant bone tumors.Methods A total of 102 patients with bone tumors diagnosed by surgery or puncture pathology and completed DCE-MRI were retrospectively analyzed.The patients were divided into benign(29 cases)and malignant(73 cases)groups.Ktrans and Kep of the two groups were calculated respectively,and the independent sample t-test was performed.The receiver operating characteristic curve was used to evaluate the diagnostic efficacy of each index.Results There were significant differences in Ktrans and Kep values between benign and malignant bone tumors(F=10.578,18.154,P<0.05).When the diagnostic cut-off points of Ktrans and Kep were 0.822(min^(-1))and 2.292(min^(-1)),the sensitivity and specificity of the diagnosis of malignant bone tumors were 89.04%and 65.75%,82.76%and 89.66%,respectively.Conclusion The quantitative parameters of DCE-MRI are helpful in differentiating benign and malignant bone tumors.
