摘  要:目的探讨3D打印技术在提高行后腹腔镜肾部分切除手术患者对肾脏解剖、手术认知程度及医师进行术前手术规划方面的应用价值。方法选取2019年3月—2022年3月在茂名市人民医院实施后腹腔镜肾部分切除术的30例患者,随机分为对照组与观察组,每组各15例。对照组术前接受双肾计算机体层血管成像(CT angiography,CTA)检查,临床医师根据影像学CT胶片进行术前评估及沟通病情拟定手术。观察组术前接受双肾CTA检查,然后将CT图像数据经过3D软件处理并打印3D模型,医师根据3D模型与患者进行沟通手术方案及手术风险,拟定具体手术计划。比较两组患者的治疗效果。结果观察组患者对肾肿瘤解剖及手术方案的认知理解明显高于对照组,差异有统计学意义(P<0.05);观察组术中出血量少于对照组,肾血流阻断时间短于对照组(P<0.05)。结论3D打印技术能帮助缩短肾血流阻断时间,减少术中出血量,能更安全高效精准地规划手术方案,可以提升患者对肾肿瘤解剖及手术方案的认识度,便于术前谈话,增强医患沟通的便利,具有较高的临床价值,值得临床推广应用。Objective To explore the application value of 3D printing technology in improving renal anatomy,surgical cognition and preoperative surgical planning of patients undergoing laparoscopic partial nephrectomy.Methods A total of 30 patients undergoing retroperitoneal laparoscopic partial nephrectomy in Maoming People’s Hospital from March 2019 to March 2022 were selected and randomly divided into control group and observation group,with 15 patients in each group.The control group received computerized tomography angiography(CTA)examination of both kidneys before surgery,and the clinician conducted preoperative evaluation and communicated the condition of the operation according to the CT film.The observation group received CTA examination of both kidneys before surgery,and then the CT image data was processed by 3D software and 3D models were printed.The doctors communicated with the patients about the surgical plan and surgical risks according to the 3D models,and drew up specific surgical plans.The therapeutic effect of the two groups was compared.Results The cognitive understanding of renal tumor anatomy and operation plan in observation group was significantly higher than that in control group,with statistical significance(P<0.05).The amount of intraoperative blood loss in the observation group was less than that in the control group,and the time of renal blood flow blockade was shorter than that in the control group(P<0.05).Conclusion 3D printing technology can help shorten the time of renal blood flow blockade,reduce the amount of intraoperative blood loss,make surgical plans more safe,efficient and accurate,improve patients’understanding of renal tumor anatomy and surgical plans,facilitate preoperative conversation,and enhance the convenience of doctor-patient communication.It has high clinical value and is worthy of clinical promotion and application.
