CAG化疗结合健脾益肾治疗老年急性髓系白血病的疗效

摘　　要：目的探究阿糖胞苷、阿克拉霉素、粒细胞集落刺激因子(cytarabine,aclarubicin,granulocyte colony stimulating factor,CAG)化疗结合中医健脾益肾治疗老年急性髓系白血病的疗效。方法以2017年1月—2022年1月在广州市第一人民医院确诊为急性髓系白血病(acute myeloid leukemia,AML)的60例老年患者为研究对象,将其随机分为单用化疗组和联合治疗组,每组30例。单用化疗组采用CAG化疗治疗,中西医结合组在此基础上服用健脾益肾中药汤剂进行补充治疗。比较两组治疗后的疗效及不良反应发生情况,观察两组患者骨髓恢复时间、血液指标和血液成分输入量,评价两组治疗后的中医症候积分及生活质量。结果联合治疗组总有效率、中医症候积分及癌症患者生活质量量表(functional assessment of cancer therapy-general,FACT-G)得分明显优于单独化疗组(P<0.05);联合治疗组感染、出血、胃肠道反应和骨髓抑制等不良反应发生率明显低于单独化疗组(P<0.05);联合治疗组骨髓恢复时间明显短于单独化疗组,血小板含量较单独化疗组高,而输注的红细胞与血小板数量明显低于单独化疗组(P<0.05)。结论CAG化疗结合健脾益肾治疗对老年AML患者的症状缓解可能有更好的效果,且能有效改善感染等不良反应的发生及血液制品的输注,使其拥有相对良好的生活质量。Objective To research the benefits of cytarabine,aclarubicin,granulocyte colony stimulating factor(CAG)chemotherapy combined with invigorating spleen and kidney in the treatment of senile acute myeloid leukemia.Methods A total of 60 elderly patients suffered from acute myeloid leukemia(AML)from January 2017 to January 2022 were randomly divided into single chemotherapy group and combined treatment group,with 30 cases in each group.The single chemotherapy group was treated with CAG chemotherapy,and the combined treatment group was supplemented with traditional Chinese medicine decoction for invigorating spleen and kidney.The efficacy and adverse reactions of them after chemotherapy were compared.In addition,the bone marrow recovery time,blood indexes and the blood component input of the two groups were observed.The traditional Chinese medicine symptom score and and quality of life between them were evaluated.Results The total effective rate,traditional Chinese medicine symptom score and functional assessment of cancer therapy-general(FACT-G)score of combined treatment group were significantly better than those of single chemotherapy group(P<0.05).The incidence of adverse reactions such as infection,bleeding,gastrointestinal reaction and bone marrow suppression in combined treatment group were significantly lower than those in single chemotherapy group(P<0.05).The bone marrow recovery time in the combined treatment group was significantly shorter than that in the single chemotherapy group,the platelet content was higher than that in the single chemotherapy group,and the number of red blood cells and platelets transfused in the combined treatment group were significantly lower than that in the single chemotherapy group(P<0.05).Conclusion CAG chemotherapy combined with invigorating spleen and kidney may be a better effect on the symptom relief of senile patients with AML,and can effectively improve the occurrence of adverse reactions such as infection and the infusion of blood products and give them a relatively good
