摘　　要：目的评价消化内科开展日间手术集中管理后运营效能的变化。方法回顾性收集2022年3—11月日间手术进行集中管理后,及上年同期2021年3—11月日间手术集中管理前的消化内科的工作量、疾病诊断相关分组(disease diagnosis related grouping,DRGs)效能数据、按病种分值付费(diagnosis-Intervention packet,DIP)支付结余等数据,比较日间手术集中管理前后的指标变化。结果消化内科开展日间手术集中管理后,科室DRGs总权重(relative weight,RW)从1799.19提高至2088.45,病例组合指数(case mix index,CMI)从0.84提高至0.87,时间消耗指数从0.78降至0.58,平均住院日从5.1 d降至3.9 d,费用消耗指数从0.97降至0.78,住院次均费用从13034.9元降至11674.7元,DIP支付结余为正比例由47.6%增至77.6%,差异均有统计学意义(P<0.05)。结论日间手术集中管理可以有效提高科室运行效能,促进科室高质量发展,具有推广应用价值。Objective To investigate the impacts of centralized management for ambulatory surgery in Gastroenterology department.Methods To collect data on workload,DRGs and DIP performance data of the Gastroenterology department after centralized management of ambulatory surgery from March to November 2022 and before centralized management of ambulatory surgery from March to November 2021 in the same period of the previous year,and compare the changes of these indicators.Results With the centralized management of ambulatory surgery,the total DRGs relative weight of the Gastroenterology department increased from 1799.19 to 2088.45,the CMI value increased from 0.84 to 0.87,the time consumption index decreased from 0.78 to 0.58,the average length of stay decreased from 5.1 to 3.9 days,the cost consumption index decreased from 0.97 to 0.78,the average cost of hospitalization decreased from 13034.9 to 11674.7,and the DIP positive balance increased from 47.6%to 77.6%,all with statistically significant differences.Conclusion The centralized management of ambulatory surgery can effectively improve the operational efficiency and promote the high-quality development of the department,which is worthy of promotion and application.
