摘　　要：目的探讨改良截石位在腹腔镜直肠癌手术中的应用效果。方法选取三明市第二医院2018年1月—2020年6月收治的直肠癌患者100例,随机分为对照组与研究组,每组各50例。观察两组患者双下肢的血流动力学相关指标以及相应的循环功能指标,并对两组患者术后并发症发生率和治疗满意度进行分析。结果研究组并发症发生率为4%,对照组为18%,两组对比差异有统计学意义(χ2=5.005,P<0.05)。两组患者各时间点循环功能对比,研究组显著优于对照组(P<0.05)。两组术后不同时间节点的下肢静脉血流动力学参数相比,差异有统计学意义(t=40.250,66.260,81.590,P<0.05),术后观察组水平优于对照组(P<0.05)。结论在对患者进行腹腔镜直肠癌手术治疗中,采用改良后的截石位体位方式能够有效缓解手术过程中患者下肢血流动力学指标,能够确保患者获得稳定的循环功能状态,并减少并发症的发生率。Objective To investigate the application effect of modified lithotomy position in laparoscopic rectal cancer surgery.Methods A total of 100 patients with rectal cancer admitted to Sanming Second Hospital from January 2018 to June 2020 were randomly divided into control group and research group,with 50 patients in each group.Hemodynamic-related indicators and the corresponding circulating function indicators of the two lower limbs of both groups were observed,and the postoperative complication probability and treatment satisfaction of the two groups were analyzed.Results The incidence of complications in the study group was 4%,while in the control group it was 18%.The difference between the two groups was statistically significant(χ2=5.005,P<0.05).The comparison of circulatory function between the two groups of patients at different time points showed that the study group was significantly better than the control group(P<0.05).There was a statistically significant difference in lower limb venous hemodynamic parameters between the two groups at different time points after surgery(t=40.250,66.260,81.590,P<0.05),and the postoperative observation group had better levels than the control group(P<0.05).Conclusion In laparoscopic rectal cancer surgery,the improved lithotomy position can effectively alleviate the lower extremity hemodynamic indicators during the operation,ensure that the patient obtains a stable circulatory function,and reduce complications The rate of occurrence.
