摘  要:目的检测首发精神分裂症患者血清长链非编码RNA BDNF反义物(LncRNA BDNF-AS)、脑源性神经营养因子(BDNF)信使核糖核酸(mRNA)水平,并分析二者对认知功能障碍的预测价值。方法选取2021年2月—2022年6月于本院接受治疗的85例首发精神分裂症患者,根据蒙特利尔认知评估量表(MoCA)评分将患者分成认知障碍组和认知正常组;另选取同期健康体检者40例作为对照组。收集受试者临床资料,并对首发精神分裂症患者分别进行阳性和阴性症状量表(PANSS)、汉密尔顿抑郁量表(HAMD)、简易智能精神状态检查量表(MMSE)、MoCA评分;荧光定量PCR法检测受试者血清LncRNA BDNF-AS、BDNF mRNA水平;Pearson法分析认知障碍组患者血清LncRNA BDNF-AS、BDNF mRNA水平与MMSE评分、MoCA评分的相关性;受试者工作特征曲线评估血清LncRNA BDNF-AS、BDNF mRNA水平对首发精神分裂症患者发生认知障碍的预测效能。结果与认知正常组相比,认知障碍组病程较长,MMSE评分、MoCA评分降低(P<0.05);对照组、认知正常组、认知障碍组血清LncRNA BDNF-AS水平依次升高,BDNF mRNA水平依次降低(P<0.05);认知障碍组患者血清LncRNA BDNF-AS水平与MMSE及MoCA评分均成负相关(P<0.05),BDNF mRNA水平与MMSE及MoCA评分均成正相关(P<0.05);血清LncRNA BDNF-AS、BDNF mRNA、两者联合预测首发精神分裂症患者发生认知障碍的曲线下面积分别为0.785、0.908、0.956。结论首发精神分裂症发生认知功能障碍患者血清LncRNA BDNF-AS成高表达,BDNF mRNA成低表达,二者与MMSE及MoCA评分有关,两者联合对首发精神分裂症患者发生认知功能障碍有较好的预测效能。Objective This paper aims to detect the serum levels of long non-coding RNA brain-derived neurotrophic factor antisense(LncRNA BDNF-AS)and brain-derived neurotrophic factor(BDNF)messenger ribonucleic acid(mRNA)in patients with first-episode schizophrenia,and to analyze their predictive value for cognitive dysfunction.Methods A total of 85 first-episode schizophrenia patients treated in our hospital from February 2021 to June 2022 were selected and divided into cognitive impairment group and normal cognitive group according to the Montreal Cognitive Assessment(MoCA)score.Another 40 healthy subjects were selected as the control group.The clinical data of the subjects were collected,and the Positive and Negative Symptom Scale(PANSS),Hamilton Depression Scale(HAMD),Mini-Mental State Examination(MMSE)and MoCA scores of the patients with first-episode schizophrenia were assessed;serum LncRNA BDNF-AS and BDNF mRNA levels of subjects were detected by fluorescence quantitative PCR;Pearson method was used to analyze the correlation between serum LncRNA BDNF-AS and BDNF mRNA levels and MMSE score and MoCA score in patients with cognitive impairment group;receiver operating characteristic curve was used to evaluate the predictive efficacy of serum LncRNA BDNF-AS and BDNF mRNA levels on cognitive impairment in first-episode schizophrenia patients.Results Compared with the normal cognitive group,the course of disease in the cognitive impairment group was longer,and the MMSE score and MoCA score were lower(P<0.05);the serum levels of LncRNA BDNF-AS in the control group,cognitive normal group and cognitive impair⁃ment group were successively increased,and the levels of BDNF mRNA were successively decreased(P<0.05);serum LncRNA BDNF-AS levels were negatively correlated with MMSE and MoCA scores in patients with cognitive impairment group(P<0.05),and BDNF mRNA levels were positively correlated with MMSE and MoCA scores(P<0.05);the areas under curve of serum LncRNA BDNF-AS,BDNF mRNA and their combination in predicting cognitive i
