改良外眦韧带及眼轮匝肌缩短术治疗退行性下睑内翻合并眼睑松弛的临床疗效与美学效果探究

摘　　要：目的:探究临床上关于退行性下睑内翻合并眼睑松弛的患者经过改良外眦韧带及眼轮匝肌缩短术治疗后,分析比较临床展现的治疗疗效以及美学效果。方法:选取2019年8月-2022年8月在笔者医院接受改良外眦韧带及眼轮匝肌缩短术的45例(86眼)患者作为改良组,按1∶1比例收集同期进行常规眼轮匝肌缩短术的45例(80眼)患者作为对照组。对改良组及对照组进行临床疗效的比较、同时评估美学效果以及比较两组患者术后并发症发生情况。结果:改良组临床疗效的总有效率为98.00%,对照组的总有效率为87.50%,改良组显著高于对照组,差异有统计学意义(P<0.05)。术后12周,改良组睑裂宽度(32.12±1.11)mm,短于对照组(34.34±1.23)mm,差异有统计学意义(P<0.05)。改良组睑裂角(51.28±2.07)°,大于对照组的睑裂角(48.12±1.75)°;睑裂倾斜度为(10.11±0.38)°,大于对照组睑裂倾斜度的(8.41±0.29)°,差异有统计学意义(P<0.05)。改良组术后并发症的发生概率为2.00%,显著低于对照组的术后并发症概率16.67%,差异有统计学意义(P<0.05)。结论:应用改良外眦韧带及眼轮匝肌缩短术治疗退行性下睑内翻合并眼睑松弛的患者,治疗后的临床疗效更好,不仅改善眼部美学效果,而且可以降低并发症的发生概率,是值得推广的手术方式。Objective To explore the clinical treatment of degenerative lower eyelid entropion with blepharochalasis by modified lateral canthal ligament and orbicularis oculi muscle shortening,and to analyze and compare the clinical therapeutic efficacy and aesthetic effect.Methods 45 patients(86 eyes)who underwent modified lateral canthus ligament and orbicularis oculi muscle shortening in the author’s hospital from August 2019 to August 2022 were selected as modifi ed group,and 45 patients(80 eyes)who received conventional orbicularis oculi muscle shortening during the same period were enrolled as control group at a 1∶1 ratio.The clinical efficacy was compared between modified group and control group,and the aesthetic eff ect was evaluated and the occurrence of postoperative complications were compared between the two groups.Results The total effective rate of clinical efficacy in modified group was 98.00% and that in control group was 87.50%.The effective rate in modifi ed group was significantly higher than that in control group(P<0.05).At 12 weeks after surgery,the width of palpebral fissure with(32.12±1.11)mm in modified group was shorter than(34.34±1.23)mm in control group(P<0.05),and the palpebral fissure angle with(51.28±2.07)°was larger than(48.12±1.75)°in control group,and the palpebral fi ssure inclination with(10.11±0.38)°was significantly larger than(8.41±0.29)°in control group(P<0.05).The incidence rate of postoperative complications in modified group was 2.00%,which was significantly lower than 16.67%in control group(P<0.05).Conclusion The application of modified lateral canthus ligament and orbicularis oculi muscle shortening has good clinical efficacy in the treatment of degenerative lower eyelid entropion with blepharochalasis.It can not only improve the eye aesthetic effect,but also reduce the incidence rate of complications,and it is a surgical method worth promoting.
