眉下切口上睑松弛矫正术皮肤去除量的准确测算方法

摘　　要：目的:探讨采用眉下切口矫正术矫正上睑皮肤松弛,皮肤去除量的精确测算方法。方法:就医者取端坐位,贴眉下缘勾画出切口上线,该线中外1/3交界处标记为a点,然后用无齿镊夹持皮肤,使睫毛微微上翘,与a点重叠的夹捏点即确定为b点,通过b点画出切口下线,a、b之间为最大去皮宽度。卧位局麻后,通过缝合a、b点模拟去皮效果,以睫毛微微翘起或者上睑微微闭合不全为标准,将新缝合点标记为b’点。经b’点画出切口下线,两线间为新的去皮范围。结果:术后随访3~6个月,57例就医者上睑松弛均有效改善,94.73%的就医者改善情况好,5.17%的就医者改善情况一般。54例就医者对手术效果满意,3例基本满意。未出现视物模糊、上睑闭合不全、瘢痕增生等并发症。结论:在夹持法测量上睑皮肤松弛的基础上,结合缝合法,可以准确判断上睑皮肤松弛的去皮量,提高上睑皮肤松弛矫正术的满意度。Objective To explore an accurate measurement method of peeling amount for correcting upper eyelid skin relaxation by eyebrow lower edge incision.Methods In the sitting position,drawing a line close to the lower edge of the eyebrow as the upper line of the incision,and marking the junction of the middle and outer 1/3 of the incision line as point a.Then use toothless forceps to grip the skin,causing the eyelashes to slightly curl up.The pinch point that overlaps with point a is determined as point b.Drawing the lower line of the incision through b point,the maximum peeling width is between points a and b.After lying flat and receiving local anesthesia,the patient was sutured at points a and b to simulate the removal effect.The new suture point was marked as point b'based on the criteria of slightly raised eyelashes or slightly incomplete closure of the upper eyelid.Draw the lower line of the incision through point b',with a new resection area between the two lines.Results The 57 patients were followed up for 3 to 6 months,the upper eyelid laxity was eff ectively improved,94.73% of the patients had good improvement,5.17%of the patients had general improvement.54 patients were satisfied with the operation results,and 3 patients were basically satisfied.There were no complications such as blurred vision,incomplete upper eyelid closure and scar hyperplasia.Conclusion Based on the measurement of upper eyelid skin relaxation by clamping method and combined with suture method,the peeling amount of upper eyelid skin relaxation can be measured accurately and the satisfaction of upper eyelid skin relaxation correction can be improved.
