摘 要:目的:探讨主动循环呼吸训练联合肺功能康复对肺癌术后患者肺功能的影响。方法:选取2019年12月至2020年12月巴中市中心医院收治的肺癌手术患者148例作为研究对象,采用随机数字表分为对照组(n=74)和研究组(n=74),对照组术后给予肺功能康复训练,研究组在对照组干预基础上联合主动循环呼吸训练,观察2组入院前和训练后7 d的肺功能指标[肺活量(vital capacity,VC)、第1s用力呼气容积(forced expiratory volume in one second,FEV1)、用力肺活量(forced vital capacity,FVC)、FEV1/FVC、每分钟最大通气量(maximum minute ventilation,MMV)、呼吸频率],活动能力指标[6分钟步行试验(6-minute walk test,6MWT)、Borg呼吸困难评分(Borg评分)]及并发症发生情况。结果:训练后研究组的VC、FEV1、FVC、FEV1/FVC、MMV水平高于对照组,呼吸频率低于对照组;研究组和对照组在训练前后的VC、FEV1、FVC、FEV1/FVC、MMV水平和呼吸频率之间均存在统计学差异(均P<0.05),2组均表现为训练后VC、FEV1、FVC、FEV1/FVC水平和呼吸频率升高,MMV水平降低。训练后2组患者活动能力指标均存在统计学差异(均P<0.05),具体表现为研究组的6MWT高于对照组,Borg评分低于对照组;研究组和对照组在训练前后的6MWT和Borg评分之间均存在统计学差异(均P<0.05)。研究组并发症发生率低于对照组(6.8%vs 18.9%),差异有统计学意义(P<0.05)。结论:主动循环呼吸训练联合肺功能康复可有效改善肺癌术后患者的肺功能,有利于提高患者活动能力,且可减少并发症,值得临床推广。Objective:To explore the effects of active cycle of breathing technique(ACBT)combined with pulmonary rehabilitation on postoperative lung functions of patients undergoing lung cancer surgery.Methods:148 patients with lung cancer treated in Bazhong Cen-tral Hospital from December 2019 to December 2020 were selected as the research objects.They were randomly assigned to the control group(n=74)and the research group(n=74)by using a random number table.The control group was given pulmonary rehabilitation after operation,and the research group was given ACRT and pulmonary rehabilitation.The changes in pulmonary function indexes[vital capac-ity(VC),forced expiratory volume in one second(FEV1),forced vital capacity(FVC),FEV1/FVC,maximum minute ventilation(MMV)and respiratory rateJ and physical ability indexes[6-minute walk test(6MWT)and Borg dyspnea score]were compared between the two groups before admission and 7 days afer training.And complications were compared between the two groups.Results:After training,the levels of VC,FEV1,FVC,FEV1/FVC and MMV in the research group were significantly higher than those in the control group(P<0.05),and the respiratory rate in the former was significantly lower than that in the latter(P<0.05);the levels of VC,FEV1,FVC,FEV1/FVC and respiratory rate in both groups increased afer training,while the MMV level decreased.After training,6MWT in the research group was significantly higher than that in the control group,and the Borg dyspnea score in the latter was significantly lower than that in the latter(all P<0.05);there were statistically significant dfferences in 6MWT and the Borg dyspnea score in both groups before and after training(all P<0.05).The incidence of complications in the research group was significantly lower than that in the control group(6.8%vs 18.9%,P<0.05).Conclusion:The use of ACBT combined with pulmonary rehabilitation can effectively improve the lung functions of patients undergoing lung cancer surgery,which helps to improve patients'physical ability and reduce com
