浓缩生长因子联合骼瑞骨粉在上颌窦外提升同期种植术中应用的临床观察

摘　　要：目的:探讨浓缩生长因子(concentrate growth factors,CGF)联合骼瑞骨粉在上颌窦外提升同期种植术中的临床应用价值。方法:选择98例上颌后牙缺失且牙槽骨剩余骨高度≤5 mm的患者,随机分为两组,每组49例。两组均给予上颌窦外提升术并行同期种植治疗,观察组术中联合给予CGF和骼瑞骨粉的混合物填充,对照组单用骼瑞骨粉填充。比较两组术后2年种植体留存率以及手术并发症发生情况。测量两组骨密度和骨高度,种植体稳定系数(implant stability quotient,ISQ)值。术后使用视觉模拟评分量表(visual analogue scale,VAS)对两组主观疼痛进行评估。结果:两组术后随访2年所有种植体均无脱落,存留率100%。观察组术中2例黏膜穿孔,对照组3例黏膜穿孔,但穿孔直径均较小。观察组术后3、6个月骨密度值均低于同期对照组(P<0.05)。观察组术后6个月及1、2年骨高度均高于同期对照组(P<0.05)。观察组术后3、6个月及1年ISQ值均高于同期对照组(P<0.05)。观察组术后3、10 d疼痛VAS评分均低于同期对照组(P<0.05)。结论:CGF联合骼瑞骨粉应用于上颌窦外提升同期种植术中能在短时间内促进新骨形成,提高种植体稳定性,减轻患者术后疼痛程度,维持骨高度,减少骨丢失。Objective:To investigate the clinical application value of concentrated growth factor(CGF)combined with iliorui bone powder in simultaneous implantation outside maxillary sinus.Methods:A total of 98 patients with maxillary poste⁃rior tooth loss and alveolar residual bone height(≤5 mm)were randomly divided into two groups with 49 cases in each group.Both groups were given external maxillary sinus lifting and concurrent implantation treatment.The observation group was com⁃bined with CGF and iliorui bone meal,while the control group was only filled with iliorui bone meal.The retention rate of im⁃plants and the incidence of surgical complications were compared between the two groups at 2 years after surgery.Bone miner⁃al density and bone height were measured in both groups,and the implant stability quotient(ISQ)was measured.The visual analogue scale(VAS)was used to assess subjective pain in both groups after surgery.Results:The two groups were followed up for 2 years after surgery,all the implants were not shed,and the implant retention rate was 100%.There were 2 cases of mucosal perforation in the observation group and 3 cases in the control group,but the perforation diameter was small.The bone mineral density of the observation group was significantly lower than that of the control group at 3 and 6 months after operation(P<0.05).The bone height of the observation group was significantly higher than that of the control group at 6 months,1,2 years after surgery(P<0.05).ISQ values in the observation group were significantly higher than those in the control group at 3,6 months and 1 year after surgery(P<0.05).The pain VAS scores of the observation group at 3 and 10 days after surgery were significantly lower than those of the control group(P<0.05).Conclusion:CGF combined with iliogrui bone meal can promote the formation of new bone in a short time,improve implant stability,reduce postoperative pain,maintain bone height,and reduce bone loss in external maxillary sinus lifting concurrent implantation.
