摘  要:目的:探讨口腔感染病原微生物检验、耐药性,并检测耐消毒剂基因。方法:回顾性分析2020年9月~2022年12月我院收治的126例口腔感染患者一般临床资料(感染组),记录其口腔病原微生物检验及耐药性结果,并对主要病原菌耐消毒剂基因进行检测分析。结果:126例患者共检出185株病原菌,革兰氏阴性菌有60株,占比32.43%,其中伴放线放线杆菌、牙龈卟啉单胞菌、福赛坦菌占比高于其他病原菌;革兰氏阳性菌有110株,占比59.46%,其中金黄色葡萄球菌、口腔链球菌、凝固酶阴性葡萄球菌占比高于其他病原菌;真菌15株,占比8.11%。伴放线放线杆菌、牙龈卟啉单胞菌、福赛坦菌对头孢曲松耐药性较高,对亚胺培南敏感;金黄色葡萄球菌、口腔链球菌、凝固酶阴性葡萄球菌均对红霉素、苯唑西林、庆大霉素耐药,对万古霉素、替考拉宁敏感。金黄色葡萄球菌、口腔链球菌、凝固酶阴性葡萄球菌qacA/B,smr基因检出率均高于伴放线放线杆菌、牙龈卟啉单胞菌、福赛坦菌。结论:革兰氏阳性菌为口腔感染主要致病菌,对红霉素、苯唑西林、庆大霉素耐药,同时携带消毒剂耐药qacA/B,smr基因,警示临床需加强管理消毒剂及抗生素药物的使用。Objective:To explore the detection and resistance of pathogenic microorganisms in oral infections,as well as the detection of disinfectant resistant genes.Methods:A retrospective analysis was conducted on the general clinical data(infection group)of 126 patients with oral infections admitted to our hospital from Sep 2020 to Dec 2022.The results of oral pathogenic microbial testing and drug resistance were recorded,and the main pathogenic bacteria’disinfectant resistance genes were monitored and analyzed.Results:Totally 185 strains of pathogenic bacteria were detected in 126 patients,of which 60 strains were Gram negative bacteria,accounting for 32.43%.Among them,Actinobacillus actinomycetes,Porphyromonas gingivalis and Fusetan bacteria accounted for more than other pathogenic bacteria.There were 110 Gram positive bacteria,ac⁃counting for 59.46%,among which Staphylococcus aureus,oral streptococcus and coagulase negative Staphylococcus account for a higher proportion than other pathogenic bacteria,and 15 fungi,accounting for 8.11%.Actinobacillus actinomycetes,Por⁃phyromonas gingivalis and Fusetan bacteria were highly resistant to ceftriaxone and sensitive to imipenem.Staphylococcus au⁃reus,oral streptococcus and coagulase negative Staphylococcus were resistant to erythromycin,oxacillin and gentamicin,and sensitive to vancomycin and teicoplanin.The detection rates of qacA/B and smr genes of Staphylococcus aureus,oral strepto⁃coccus,coagulase negative Staphylococcus were higher than those of Actinobacillus actinomycetes,Porphyromonas gingivalis and Fusetan bacteria.Conclusion:Gram positive bacteria are the main pathogenic bacteria of oral infection.They are resistant to erythromycin,oxacillin and gentamicin,and carry the qacA/B and smr genes of disinfectant resistance.It warns that clinical management of the use of disinfectants and antibiotics should be strengthened.
