摘  要:目的探讨非动脉炎性前部缺血性视神经病变(NAION)患者功能核磁共振成像(BOLD-fMRI)的变化特征。方法纳入2018年1月—2019年1月就诊于北京中医药大学东方医院眼科的NAION患者20例作为观察组。同时纳入20例性别、年龄相匹配的同医院同时期体检的健康人作为对照组。所有对象入组后,均进行BOLD-fMRI检查,选用静息态的数据处理软件进行数据处理及分析,获得BOLD-fMRI大脑低频振幅(ALFF),用峰值T表示,总结NAION患者BOLD-fMRI的变化特征。结果与对照组比较,观察组初级视觉皮层(即布罗德曼17区)、右脑岛、额中回区域的峰值T均为负值,表明ALFF降低;观察组脑桥、舌状回、楔前叶、中央旁小叶区域的峰值T均为正值,表明ALFF增加。结论NAION患者部分脑区功能活动异常,默认网络(DMN)功能障碍,这可能利于NAION的早期诊断。OBJECTIVE To explore the characteristics of changes in functional magnetic resonance imaging(BOLD-fMRI)in patients with non-arteritic anterior ischemic optic neuropathy(NAION).METHODS A total of 20 cases with NAION who were treated in the Department of Ophthalmology at the Dongfang Hospital of Beijing University of Chinese Medicine from January 2018 to January 2019 were included as the observation group.A total of 20 healthy individuals who underwent physical examinations during the same period at the same hospital,matched for gender and age,were included as the control group.After enrollment,all subjects underwent BOLD-fMRI examination.Resting-state data processing software was used for data analysis,and the amplitude of low-frequency fluctuations(ALFF)of the BOLD-fMRI signal,expressed as peak T-values,were obtained to summarize the characteristics of BOLD-fMRI changes in patients with NAION.RESULTS Compared with the control group,the observation group showed negative peak T-values in the primary visual cortex(Brodmann area 17),the right insula,and the frontal middle gyrus,indicating decreased ALFF.The observation group showed positive peak T-values in the pons,lingual gyrus,precentral gyrus,and paracentral lobule,indicating increased ALFF.CONCLUSIONS The patients with NAION exhibit abnormal functional activity in certain brain regions and dysfunction of the default mode network(DMN),which may contribute to the early diagnosis of NAION.
