摘  要:目的分析孤独症谱系障碍(ASD)儿童与正常儿童不同脑区弛豫时间的差异,探索集成MRI技术(Synthetic MRI,SyMRI)在评估ASD儿童大脑微结构异常方面的应用价值。方法纳入ASD儿童30例(ASD组),健康儿童20例(对照组),所有受试者均在3.0 T MRI上进行常规MRI和集成MRI序列MAGiC的扫描,经过后处理获取所有受试者胼胝体膝部、胼胝体压部及顶叶白质、额叶白质、内囊后肢、内囊前肢、丘脑、苍白球、尾状核头、壳核的T_(1)、T_(2)弛豫值。分析ASD组与对照组各个脑区T_(1)、T_(2)弛豫值之间的差异。评价ASD组有差异脑区的T_(1)、T_(2)弛豫值与儿童发育行为能力的相关性。探讨T_(1)、T_(2)弛豫值对儿童ASD的诊断效能。结果与对照组相比,ASD组右侧内囊前肢T_(1)弛豫值、胼胝体膝部和左侧内囊后肢T_(2)弛豫值均降低(P<0.05)。在Gesell量表评分中,ASD组右侧内囊前肢T_(1)弛豫值与大运动发育商呈正相关(r=0.385,P<0.05),胼胝体膝部、左侧内囊后肢T_(2)弛豫值与个人社交行为发育商分别呈正相关、负相关(r=0.445、-0.372,P<0.05);ASD组右侧内囊前肢T_(1)弛豫值、胼胝体膝部和左侧内囊后肢T_(2)弛豫值与CARS评分均无显著相关性(P>0.05)。ASD组右侧内囊前肢T_(1)弛豫值、胼胝体膝部和左侧内囊后肢T_(2)弛豫值的AUC分别为0.682、0.718和0.695。结论ASD儿童部分脑区弛豫时间异常,反映出大脑微结构异常,集成MRI技术在评估ASD儿童大脑微结构异常方面具有一定价值。Objective To observe the differences of relaxation time in different brain regions in children with Autism Spectrum disorder(ASD),and to explore the application value of Synthetic MRI(SyMRI)in the evaluation of brain microstructure abnormalities in children with ASD.Methods 30 children with ASD(ASD group)and 20 healthy children(control group)were included.Data of conventional and synthetic MRI sequence were obtained on 3.O T MRI for all subjects.After post-processing,T_(1)and T_(2)relaxation time of genu of the corpus callosum,splenium of the corpus callosum,parietal white matter,frontal white matter,posterior limb of internal capsule,anterior limb of internal capsule,thalamus,globus pallidus,head of caudate nucleus and putamen were obtained.The differences of relaxation time in different brain regions between ASD group and control group were investigated.To evaluate the correlation between the T_(1)and T_(2)relaxation times of different brain regions and the developmental behavioral ability of children in ASD group.To investigate the diagnostic efficacy of T_(1)and T_(2)relaxation times in children with ASD.Results T_(1)relaxation time in the right anterior limb of internal capsule,T_(2)relaxation times in the genu of the corpus callosum and the left posterior limb of internal capsule in the ASD group were decreased than in the control group(all P<0.05).Among Gesell scale scores,the T_(1)relaxation time of the right anterior limb of internal capsule was positively correlated with the gross motor development quotient in ASD group(r=O.385,P<0.05),the T_(2)relaxation times in genu of the corpus callosum and left posterior limb of internal capsule were positively and negatively correlated with the developmental quotient of personal social behavior respectively(r=0.445、-0.372,all P<0.05).There was no significant correlation between the T_(1)relaxation time of right anterior limb of internal capsule and the T_(2)relaxation times of genu of the corpus callosum and left posterior limb of internal capsule and the CARS
