针刺对脑瘫神经功能修复和脑部微循环改善作用机制

摘　　要：小儿脑瘫(cerebral palsy,CP)发病率高,不仅严重影响患儿生长发育、生活质量,也给家庭和社会带来了沉重的负担,前期研究发现针刺对于脑瘫疗效显著,并且可以从多个方面干预CP的治疗,但目前对于针刺治疗CP的机制尚无明确和固定的结论,该研究针刺干预CP主要从脑神经损伤的修复,脑部微循环的改善以及两者的相互关系出发,对影响其有关方面的机制进行综述。该文发现针刺能够激活大脑皮质相关功能区,改善皮质小脑的连接,调控锥体系皮层之间的传导,调节神经生长相关蛋白和神经细胞因子的表达,以及调控神经递质释放,改变脑血流,调控相关通路系统和自主神经等环节来恢复脑神经损伤和改善脑部血液循环,从而有效地改善CP患儿临床症状,对今后CP的临床针刺治疗具有重要的指导意义。The incidence of cerebral palsy(CP)in children is high,which not only seriously affects the growth and development of children and the quality of life,but also brings a heavy burden to the family and society.Preliminary some studies have found that acupuncture has obvious therapeutic effect on CP and can intervene in the treatment of CP from many aspects.However,there is no clear and fixed conclusion on the mechanism of acupuncture treatment of CP.In this study,acupuncture intervention for CP from the repair of cranial nerve damage,the improvement of brain microcirculation,and the relationship between the two,about the mechanisms affecting their related aspects are reviewed.This paper found that acupuncture can activate the relevant functional areas of the cerebral cortex,improve the connection between the cortex and cerebellum,regulate the conduction between the pyramidal cortex,regulate the expression of nerve growth-related proteins and neurocytokines,as well as the content of neurotransmitters releases,change cerebral blood flow,regulate the relevant pathways and autonomic nerves,the effect of restore cranial nerve damage and improve blood circulation in the brain can effectively improve the clinical symptoms of children with CP,which has important guiding significance for the clinical acupuncture treatment of CP in the future.
