摘 要:肝细胞癌(hepatocellular carcinoma,HCC)是临床常见的恶性肿瘤之一,该病具有起病隐匿、恶化率高、预后极差等特点,临床早期诊断和治疗缺乏特异性分子标志物。近年来越来越多的调节性细胞死亡(regulated cell death,RCD)方式逐渐被认识,研究者发现多种RCD在HCC发生、发展的过程中发挥着促癌和抑癌的双重作用,不同RCD之间能够相互作用、相互调控,早期阶段通过促进炎症反应、纤维化形成和血管侵袭以加重对肝细胞的损伤,但晚期阶段却能提高抗癌药物的敏感性以限制肿瘤生长。基于肿瘤微环境(tumor microenvironment,TME)的RCD靶向干预治疗也日趋成为研究热点,目前针对HCC不同阶段的治疗方案已有不同的RCD靶向抗癌药物。该文着重介绍了凋亡、坏死性凋亡、焦亡、自噬和铁死亡等不同类型RCD调控HCC作用机制的研究进展,归纳总结了RCD对TME的靶向干预作用,以期为深入研究HCC中RCD的复杂调控机制和临床靶向治疗药物的研发应用提供理论参考和指导。Hepatocellular carcinoma(HCC)is one of the most common malignant tumors in clinic.HCC is characterized by insidious onset,high deterioration rate and very poor prognosis.Early clinical diagnosis and treatment lack specific molecular markers.In recent years,more and more regulated cell death(RCD)modes have been gradually recognized.Researchers have found that a variety of RCDs play a dual role in promoting cancer and tumor suppression in the process of HCC development.Different RCDs can interact and regulate each other.Early stage aggravates liver cell injury by promoting inflammatory response,fibrosis formation and vascular invasion,but late stage can improve the sensitivity of anticancer drugs to limit tumor growth.RCD targeted intervention therapy based on tumor microenvironment(TME)has also increasingly become a research hotspot,and there are different RCD targeted anticancer drugs for different stages of HCC.In this paper,the mechanism of RCD in regulating HCC,such as apoptosis,necroptosis,pyroptosis,autophagy and ferroptosis,is introduced emphatically,and the targeted intervention effect of RCD on TME is summarized in order to provide theoretical reference and guidance for in-depth study of the complex regulatory mechanism of RCD in HCC and the research and development and application of clinical targeted therapeutics.
