摘　　要：目的探讨儿童后肾腺瘤的CT和MRI表现。资料与方法回顾性分析2011年11月—2021年8月上海市儿童医院经手术病理证实的6例后肾腺瘤患儿,观察其病灶位置、大小及形态、CT和MRI特征。结果病变均为单发,右肾1例,左肾5例,肿瘤最大直径约1.6~9.7 cm,平均(4.1±3.0)cm。5例呈类圆形或椭圆形,1例呈8字形。CT显示4例呈低密度,1例呈等密度,1例呈混杂密度,5例病灶以实性为主肿块,1例为囊性为主肿块,其中1例伴有斑片状钙化,1例伴有出血;增强CT扫描显示病灶边界清晰,实性成分均呈渐进性轻中度强化,强化程度低于正常肾实质。MRI显示4例实性成分均可见扩散受限。结论儿童后肾腺瘤的影像学表现具有一定的特征性,病灶单发,边界清楚,实性成分均呈渐进性轻中度强化且扩散受限,需警惕本病的可能。Purpose To investigate the CT and MR imaging features of metanephric adenoma(MA)in children.Materials and Methods A total of 6 cases pathologically diagnosed as MA in Shanghai Children’s hospital from November 2011 to August 2021 were retrospectively selected,and CT and MR imaging characteristics of MA were also reviewed in this study,the location,size and morphology,CT and MRI features of the lesion were observed.Results All MA lesions were single,five cases located in the left kidney and one case in the right kidney,tumor diameter was about 1.6~9.7 cm,the average was(4.1±3.0)cm.Five MA lesions were round or ellipse and one lesion was“8”glyph.CT showed low density in 4 cases,isodensity in 1 case,mixed density in 1 case,solid mass in 5 cases,cystic mass in 1 case,including 1 case with patchy calcification and 1 case with hemorrhage.Enhanced CT scan showed clear boundary of the lesion and progressive mild to moderate enhancement of solid components,which was lower than normal renal parenchyma.MRI showed limited diffusion in all 4 solid components.Conclusion MA has some imaging characteristics,including the single well-defined lesion,persistently mild to moderate enhancement solid component,high signal on diffusion weighted imaging,and low signal on apparent diffusion coefficient,which may need to be alert to the MA in children.
