^(14)C尿素呼气试验在诊断幽门螺杆菌感染中的临床价值

摘　　要：目的分析^(14)C尿素呼气试验(^(14)C-UBT)在幽门螺杆菌(Hp)感染诊断中的应用价值。方法本项研究于2019年6月正式启动,截止时间是2022年6月。选用此时间段到医院接受就诊的上腹部不适患者200例为研究对象,均接受快速尿素酶试验(RUT)和^(14)C-UBT,以活体组织检查为诊断金标准,对比观察两组诊断方法应用价值。结果200例患者中,经病理检查确诊有181例,比较两组检测方法对慢性胃炎、十二指肠炎、消化性溃疡等疾病的诊断阳性率,对比两组检测方法对胃癌、胃息肉、慢性胃炎的阳性检出率,结果无显著差异(P>0.05);在十二指肠炎、消化性溃疡阳性率诊断上,^(14)C-UBT阳性检出率高,组间数据比较差异具有统计学意义(P<0.05);比较两组诊断方式的诊断效能,包括对疾病的诊断灵敏度、准确度等,^(14)C-UBT检测方法具有更好的诊断效能(P<0.05)。结论^(14)C-UBT和RUT是诊断HP感染的有效检测手段,但前者能有效提高Hp检出率,且诊断效能较为显著,能够为患者后续诊治提供更多的指导依据,临床值得推广应用。Objective To analyze application value of ^(14)C urea breath test(^(14)C UBT)in diagnosis of Helicobacter pylori(Hp)infection.Methods The study was officially launched in June 2019 with deadline of June 2022,chose 200 patients with epigastrium discomfort in the hospital during the period as study objects,both groups were treated with rapid urease test(RUT)and ^(14)C-UBT.With diagnostic gold standard of live tissue examination,application value between two diagnostic methods was compared.Results Among the 200 patients,181 were confirmed through pathological examination.Positive diagnostic rates comparison of chronic gastritis,duodenitis,peptic ulcer and other diseases between two test methods showed,there was no difference in positive detection rates of gastric cancer,gastric polyps and chronic gastritis between two test methods(P>0.05).In diagnosis of duodenitis and peptic ulcer,positive detection rate of ^(14)C-UBT was higher,data comparison difference between groups was statistically significant(P<0.05).Diagnostic effect comparison between two groups of diagnostic methods showed,including data difference in diagnostic sensitivity and accuracy for diseases,the ^(14)C-UBT detection method had better diagnostic effect(P<0.05).Conclusion ^(14)C-UBT and RUT is effective detection method for diagnosing Hp infection.However,the former can improve detection rate of Hp effectively with significant diagnostic effect,provide more guidance for subsequent diagnosis and treatment of patients,which is worthy of promotion and application in clinical practice.
