鞍结节脑膜瘤两种手术方法的探讨

摘　　要：目的探讨眶上外侧入路和单侧额底入路在鞍结节脑膜瘤手术治疗中的临床效果及安全性。方法收集福建省立医院2010年1月至2020年12月经手术及术后病理确诊的鞍结节脑膜瘤51例,回顾性分析患者的临床资料,将51例患者分为两组,单侧额底入路显微手术切除肿瘤(额底组)22例,眶上外侧入路显微手术切除肿瘤(眶外组)29例,比较两组患者手术时间、住院时间、肿瘤切除程度、术后视力变化情况,评估两种手术方法的安全性及手术效果。结果肿瘤切除程度、术后视力改善情况在额底组和眶外组中差异无统计学意义(P>0.05),眶外组患者在手术时间、住院时间等方面均优于额底组,两组差异有统计学意义(P<0.05)。结论经眶上外侧入路显微手术切除鞍结节脑膜瘤,具有开关颅简单,组织创伤小,术野清晰足够,手术效果良好等优点,是一种微创、安全、有效的手术方法。Objective To assess the clinical effects and safety of lateral supraorbital approach and unilateral subfrontal approach applied in microsurgery for tuberculum sellae meningiomas.Methods A total of 51 cases of saddle nodular meningioma diagnosed by surgery and postoperative pathology from January 2010 to December 2020 in Fujian Provincial Hospital were collected,and the clinical data of the patients were retrospectively analyzed,and 51 patients were divided into two groups,22 cases of unilateral frontal floor surgical resection(frontal floor group)and 29 cases of surgical resection of supralateral orbital approach(extraorbital group),compared the operation time,hospital stay,tumor resection degree and postoperative visual acuity changes of the two groups,and evaluated the safety and surgical effect of the two surgical methods.Results There were no significant differences in the degree of tumor resection and postoperative visual acuity improvement between the frontal fundus group and the extraorbital group(P>0.05),and the patients in the extraorbital group were better than the frontal fundus group in terms of operation time and hospital stay,and the difference between the two groups was statistically significant(P<0.05).Conclusion Lateral supraorbital approach applied in microsurgery for tuberculum sellae meningiomas has the advantages of simple craniotomy,samll tissue trauma,clear and sufficient operation field and good operation effect.It is a minimally invasive,safe and effective operation method.
