摘 要:目的探究血清白细胞分化抗原30(CD30)、B细胞活化因子(BAFF)、可溶性肿瘤坏死因子1受体(sTNF-R1)、趋化因子10(CXCL10)表达水平与双相障碍不同情绪状态的相关性。方法选取确诊为双相障碍的患者80例,根据不同情绪状态分为躁狂发作组50例与抑郁发作组30例,并选取健康个体40例作为对照组。采用酶联免疫吸附剂测定法测定血清CD30、BAFF、sTNF-R1、CXCL10水平,并采用汉密尔顿抑郁量表和杨氏躁狂量表评估患者临床症状。结果与健康对照组相比,躁狂发作组血清CD30、BAFF表达水平更高(P<0.001),CXCL10表达水平更低(P<0.001)。抑郁发作组CD30、BAFF表达水平显著高于健康对照组(P<0.001)。与抑郁发作组相比,躁狂发作组BAFF、sTNF-R1表达水平更高(P<0.01),CXCL10表达水平更低(P<0.001)。抑郁发作组CXCL10水平与HAMD总分、焦虑/躯体化因子分、认知障碍因子分呈正相关。结论CD30、BAFF可能是双相障碍诊断的生物学指标,CXCL10可能是躁狂发作诊断的生物学指标。BAFF、s TNF-R1、CXCL10可能成为双相障碍患者不同情绪状态的状态标记物。CXCL10可能反应抑郁发作患者的抑郁焦虑与认知水平。Objective To explore the correlation between the expression levels of serum CD30,BAFF,sTNF-R1,CXCL1O and different emotional states in patients with bipolar disorder.Methods Comparing concentrations of cytokines(CD30,BAFF,sTNF-R1,CXCLI0)in 80 participants with bipolar disorder diagnosed in our Hospital from September 2020 to March 2023 across 2 mood states(mania N=50,depression N=30)and 40 controls without a psychiatric disorder(total N=120)by enzyme linked immunosorbent assay.The clinical symptoms of patients were evaluated using the Hamilton Depression Scale and the Young's Mania Rating Scale.Results Compared with the healthy control,the expression levels of CD30 and BAFF in the manic patients were higher(P<0.001),while the expression levels of CXCL10 were lower(P<0.001).The expression levels of CD30 and BAFF in the depressive patients were significantly higher than those in the healthy control(P<0.001).Compared with the depressive patients,the expression levels of BAFF and sTNF-RI in the manic patients were higher(P<0.01),while the expression levels of CXCL10 were lower(P<0.001).CXCL10 in patients with depressive episodes was significantly positively correlated with total HAMD scores,anxiety/somatization factor scores,and cognitive impairment factor scores.Conclusions CD30 and BAFF may be biomarkers for the diagnosis of bipolar disorder,and CXCL10 may be biomarkers for the diag nosis of manic episodes.BAFF,sTNF-R1,and CXCLIO may become state markers for diagnosing different emotional states in patients with bipolar disorder.CXCL10 may reflect the depression,anxiety,and cognitive level of patients in bipolar disorder with depressive episodes.
