摘　　要：目的 探究Ⅴ型胶原蛋白α2(COL5A2)在乳腺癌组织中的表达及其与预后和免疫微环境的关系。方法 选取2017年1月至2020年12月于徐州市肿瘤医院行乳腺癌切除术治疗的164例乳腺癌患者作为研究对象,经手术获得其乳腺癌组织和癌旁正常组织样本。免疫组化分析组织中COL5A2的表达及其与患者临床病理特征和免疫微环境的关系;Kaplan-Meier法分析两组患者的预后情况;多因素Cox比例风险回归分析影响患者预后的因素;构建列线图模型并评价其预测效能。结果 乳腺癌组织中COL5A2阳性表达率明显高于癌旁正常组织(P<0.05);COL5A2表达与患者的组织学分级、临床分期、淋巴结转移有关(P<0.05);COL5A2高表达组患者2年无进展生存率和2年总生存率均明显低于COL5A2低表达组(P<0.05);肿瘤直径≥2cm、临床分期Ⅲ~Ⅳ期、淋巴结转移、COL5A2高表达是影响患者2年无进展生存率和2年总生存率的危险因素(P<0.05),术后新辅助化疗是患者2年无进展生存率的保护因素(P<0.05);列线图模型的区分度较高,准确性和有效性较好;COL5A2表达与乳腺癌组织中的免疫微环境改变有关。结论 COL5A2在乳腺癌组织中高表达,其高表达影响乳腺癌患者的预后和肿瘤免疫微环境。Objective To explore the expression of type V collagenα2(COL5A2)in breast cancer and its relationship with prognosis and immune microenvironment.Methods A total of 164 patients with breast cancer who underwent mastectomy in Xuzhou Cancer Hospital from January 2017 to December 2020 were selected as the subjects.The expression of COL5A2 in tissue and its relationship with clinicopathological features and immune microenvironment were analyzed by immunohistochemistry.The prognosis of the two groups was analyzed by Kaplan-Meier method.Multivariate Cox proportional hazard regression analysis was used to analyze the factors affecting the prognosis of patients.Construct the line chart model and evaluate its prediction efficiency.Results The positive expression rate of COL5A2 in breast cancer tissues was significantly higher than that in adjacent normal tissues(P<0.05).The expression of COL5A2 was related to histological grade,clinical stage and lymph node metastasis(P<0.05).The 2-year progression-free survival rate and 2-year overall survival rate of patients with high expression of COL5A2 were significantly lower than those of patients with low expression of COL5A2(P<0.05).Tumor diameter≥2 cm,clinical stage-,lymph node metastasis and high expression of COL5A2 were risk factors for 2-year progression-free survival and 2-year overall survival.Postoperative neoadjuvant chemotherapy was a protective factor for 2-year progression-free survival(P<0.05).The discrimination degree of the line chart model is high,and the accuracy and validity are good.The expression of COL5A2 is related to the changes of immune microenvironment in breast cancer.Conclusion COL5A2 is highly expressed in breast cancer,and its high expression affects the prognosis and tumor immune microenvironment of patients with breast cancer.
