乳腺原发性鳞状细胞癌超声特征与Ki-67表达的相关性研究

摘　　要：目的研究乳腺原发性鳞状细胞癌(PSCCB)超声表现与Ki-67表达的关系。方法回顾性筛选万宁市人民医院2013年10月至2022年4月经手术病理确诊的PSCCB患者29例,分析病灶超声声像图特点并与Ki-67表达情况做对比。结果PSCCB超声征象中肿瘤形态均不规则,长径≥3 cm占72.41%(21/29),低回声占65.52%(19/29),囊实混合回声占34.48%(10/29),边缘不光整占86.21%(25/29),后方回声增强占68.96%(20/29),肿块内微钙化占27.59%(8/29),平行位占79.31%(23/29),血供丰富占75.86%(22/29)。Ki-67阳性24例,长径≥3 cm、供血情况与Ki-67表达呈正相关(r1=0.724,r2=0.634,P<0.05),其余超声特点与Ki-67相关性比较,均差异无统计学意义(P>0.05)。结论PSCCB存在Ki-67高表达,其超声声像图具有一定的特征,肿块越大、血流越丰富越提示较高比例的Ki-67指数,上述信息可为临床诊疗和预后评估提供参考。Objective To investigate the relationship between ultrasonography and Ki-67 expression in primary squamous cell carcinoma of the breast(PSCCB).Methods A retrospective screening of 29 patients with PSCCB in People’s Hospital of Wanning from October 2013 to April 2022 who were diagnosed by surgical pathology and pathology were conducted,and the characteristics of ultrasonographic images of lesions were analyzed and compared with the expression of Ki-67.Results The shape of the tumor had been irregular in PSCCB ultrasonographic features,with the longest diameter 3 cm accounting for 72.41%(21/29),low echo accounting for 65.52%(19/29),mixed echo accounting for 34.48%(10/29),uneven margin accounting for 86.21%(25/29),posterior echo enhancement accounting for 68.96%(20/29),and microcalcification in the mass accounting for 27.59%(8/29).Parallel posture was responsible for 79.31%(23/29),and abundant blood was responsible for 75.86%(22/29).Ki-67 was positive in 24 cases,and the longest diameter 3 cm and blood supply were positively associated with Ki-67 expression(r1=0.724,r2=0.634,P<0.05),but no other ultrasonic indicators were significantly different(P>0.05).Conclusion Ki-67 is highly expressed in PSCCB,and its ultrasonography has specific properties.The higher the Ki-67 index,the greater the longest diameter of the mass and the richer the blood flow signal in the mass.The qualities listed above can be used as a guide for clinical diagnosis,therapy,and prognosis evaluation.
