摘 要:目的:观察健脾止泻宁颗粒联合美沙拉嗪对溃疡性结肠炎(UC)模型大鼠细胞因子以及结肠组织中髓过氧化物酶(MPO)水平的影响。方法:75只Wistar雄性大鼠采用随机数字表法随机分为5组:正常对照组、模型对照组、健脾止泻宁组、美沙拉嗪组和联合治疗组,每组15只。采用大鼠疾病活动指数(DAI)评分评估大鼠肠炎症状严重程度的指标。酶联免疫吸附试验(ELISA)检测各组大鼠血清中细胞因子白细胞介素(IL)-1β、IL-4、IL-6、IL-8、IL-10、肿瘤坏死因子-α(TNF-α)、IL-18水平,蛋白质印迹法(Western blot)检测结肠黏膜组织中MPO的表达。采用单因素方差分析进行统计学分析。结果:与正常对照组比较,模型组及各个给药治疗组DAI评分均增高,差异有统计学意义(F=128.574,P<0.05);与模型组比较,各治疗组DAI评分均降低,差异有统计学意义(F=128.574,P<0.05)。与模型组比较,健脾止泻宁组、美沙拉嗪组及联合用药组血清IL-1β、IL-6、IL-8、TNF-α和IL-18水平升高(F=90.948、92.243、132.437、37.070、61.971,P<0.05),IL-4和IL-10水平降低(F=82.250、83.657,P<0.05);与健脾止泻宁组和美沙拉嗪组比较,联合用药组血清IL-1β、IL-6、IL-8、TNF-α和IL-18水平降低(F=90.948、92.243、132.437、37.070、61.971,P<0.05),IL-4和IL-10水平升高(F=82.250、83.657,P<0.05)。与模型组比较,健脾止泻宁组、美沙拉嗪组及联合用药组结肠组织中MPO含量降低(F=178.140,P<0.05);与健脾止泻宁组和美沙拉嗪组比较,联合用药组结肠组织中MPO含量降低(F=178.140,P<0.05)。血清IL-1β、IL-6、IL-8、TNF-α、IL-18的表达水平与大鼠结肠组织中MPO的含量呈正相关(r=0.821、0.861、0.886、0.718、0.793,P<0.05),血清IL-4、IL-10的表达水平与大鼠结肠组织中MPO的含量呈负相关(r=-0.837、-0.817,P<0.05)。结论:联合使用健脾止泻宁颗粒和美沙拉嗪治疗UC可以产生协同效应。Objective To observe the effects of Jianpi Zhixiening granules combined with mesalazine on cytokines and myeloperoxidase(MPO)levels in colon tissue of ulcerative colitis(UC)model rats.Methods A total of 75 Wistar male rats were randomly divided into 5 groups using a random number table method:normal control group,model control group,Jianpi Zhixie Ning group,mesalazine group,and combined treatment group,with 15 rats in each group.The Rat Disease Activity Index(DAI)score was used to evaluate the severity of colitis symptoms in rats.Enzyme linked immunosorbent assay(ELISA)was used to detect cytokine interleukin(IL)-1β,IL-4,IL-6,IL-8,IL-10,tumor necrosis factor-α(TNF-α),IL-18 level in serum of rats in each group.Western blotting was used to detect the expression of MPO in colonic mucosal tissue.Statistical analysis was conducted using one-way ANOVA.Results The DAI scores in the model group and each treatment group were significantly higher than those in the normal control group(F=128.574,P<0.05).Compared to the model group,the DAI scores in each treatment group decreased significantly(F=128.574,P<0.05).Compared to the model group,the serum levels of IL-1β,IL-6,IL-8,TNF-αand IL-18 were elevated in the Jianpi Zhixia Ning group,Mesalazine group,and combination therapy group(F=90.948,92.243,132.437,37.070,61.971,P<0.05),while the levels of IL-4 and IL-10 were decreased(F=82.250,83.657,P<0.05).Compared to the Jianpi Zhixia Ning group and Mesalazine group,the combination therapy group showed a decrease in serum levels of IL-1β,IL-6,IL-8,TNF-α,and IL-18(F=90.948,92.243,132.437,37.070,61.971,P<0.05),and an increase in IL-4 and IL-10 levels(F=82.250,83.657,P<0.05).Compared to the model group,the Jianpi Zhixia Ning group,Mesalazine group,and combination therapy group showed a decrease in MPO content in colon tissue(F=178.140,P<0.05).Compared to the Jianpi Zhixia Ning group and Mesalazine group,the combination therapy group showed a decrease in MPO content in colon tissue(F=178.140,P<0.05).The expression levels of serum
