摘 要:目的:检测骨软骨瘤组织、骨肉瘤组织中Krüppel样因子4(KLF4)和核转运蛋白a2(KPNA2)的表达,探讨KLF4和KPNA2与骨肉瘤患者临床特征的关系。方法:选取2019年3月至2022年10月长江大学附属荆州医院和襄阳市中心医院病理确诊的36例骨软骨瘤组织和98例骨肉瘤组织标本作为研究对象,采用免疫组织化学法检测上述组织中KLF4和KPNA2的表达,结合临床资料,采用χ^(2)检验进行统计学分析。结果:骨肉瘤组织中KLF4阳性表达率54.08%(53/98),明显高于骨软骨瘤组织中KLF4阳性表达率16.67%(6/36),差异有统计学意义(χ^(2)=14.956,P<0.01)。骨肉瘤组织中KPNA2阳性表达率63.27%(62/98),明显高于骨软骨瘤组织中KPNA2阳性表达率27.78%(10/36),差异有统计学意义(χ^(2)=13.337,P<0.01)。KLF4表达水平与骨肉瘤患者恩内金分期(Eneeking分期)、肺转移、软组织浸润明显相关(χ^(2)=5.408、4.688、4.322,P<0.05),KLF4表达水平与骨肉瘤患者病变部位、性别、组织类型、ALP水平、年龄无明显相关(χ^(2)=0.001、0.115、0.164、0.027、0.023,P>0.05)。KPNA2表达水平与骨肉瘤患者Eneeking分期、肺转移明显相关(χ^(2)=5.157、5.284,P<0.05),KPNA2表达水平与骨肉瘤患者软组织浸润、病变部位、性别、组织类型、年龄、ALP水平无明显相关(χ^(2)=0.004、0.001、0.084、0.047、0.872、0.088,P>0.05)。结论:骨肉瘤组织中KLF4和KPNA2呈高表达,骨肉瘤组织KLF4和KPNA2高水平表达与预后不良相关,联合检测KLF4和KPNA2有助于判断骨肉瘤患者预后。Objective To detect the expression and clinical significance of krüppel-like factor 4(KLF4)and Karyopherin a2(KPNA2)in osteochondroma and osteosarcoma tissues,and to explore the relationship between KLF4 and KPNA2 and the clinical characteristics of osteosarcoma patients.Methods A total of 36 cases of osteochondroma tissue and 98 cases of osteosarcoma tissue specimens confirmed by pathology in Jingzhou Hospital Affiliated to Yangtze University and Xiangyang Central Hospital from March 2019 to October 2022 were selected as the research objects.The expression of KLF4 and KPNA2 in the above tissues was detected by immunohistochemistry,and Chi-square test were used for statistical analysis combined with clinical data.Results The positive expression rate of Cdc42 in osteosarcoma tissues was 62.50%(60/96),which was significantly higher than that in osteochondroma tissues[11.43%(4/35),χ^(2)=26.774,P<0.01].The positive expression rate of KLF4 in osteosarcoma tissues was 54.08%(53/98),which was significantly higher than that in osteochondroma tissues[16.67%(6/36),χ^(2)=14.956,P<0.01].The positive expression rate of KPNA2 in osteosarcoma tissues was 63.27%(62/98),which was significantly higher than that in osteochondroma tissues[27.78%(10/36),χ^(2)=13.337,P<0.01].The expression level of KLF4 was significantly correlated with Eneeking stage,lung metastasis and soft tissue invasion in osteosarcoma patients(respectively,χ^(2)=5.408,4.688,4.322,P<0.05),but not significantly correlated with the lesion location,gender,histological type,ALP level and age of osteosarcoma patients(respectively,χ^(2)=0.001,0.115,0.164,0.027,0.023,P>0.05).The expression level of KPNA2 was significantly correlated with Eneeking stage and lung metastasis of osteosarcoma patients(respectively,χ^(2)=5.157,5.284,P<0.05),but not significantly correlated with soft tissue invasion,lesion location,gender,tissue type,age and ALP level in osteosarcoma patients(respectively,χ^(2)=0.004,0.001,0.084,0.047,0.872,0.088,P>0.05).Conclusion KLF4 and KPNA2 are h
