摘　　要：目的比较不同治疗方案治疗局部晚期宫颈癌患者的近期及远期疗效。方法根据治疗方法的不同将471例局部晚期宫颈癌患者分为手术(ST)组154例、新辅助化疗加手术(NST)组249例和根治性放疗(RT)组68例。比较3组患者的复发率、近期疗效和远期疗效,并分析预后的影响因素。结果3组患者复发率、3年无病生存率、3年总生存率、5年无病生存率比较,差异均无统计学意义(P﹥0.05)。NST组患者5年总生存率高于ST组和RT组,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,治疗方式是局部晚期宫颈癌患者预后的独立影响因素(P﹤0.01)。结论3种治疗方式的复发率和近期疗效没有差异,与RT或ST相比,NST改善了局部晚期宫颈癌患者的远期疗效。Objective To compare the short-term and long-term efficacy of different treatments for locally advanced cervical cancer.Method A total of 471 patients with locally advanced cervical cancer were divided into operation group(ST group,n=154),neoadjuvant chemotherapy plus operation group(NST group,n=249)and radical radiotherapy group(RT group,n=68).The recurrence rate,short-term efficacy and long-term efficacy of the three groups were compared and the influencing factors for prognosis were analyzed.Result There were no significant differences for recurrence rate,3-year disease free survival rate,3-year overall survival rate and 5-year disease free survival rate among the three groups(P>0.05);the 5-year overall survival rate of NST group was higher than that of ST group and RT group(P<0.05);multi-variate analysis showed that the treatment method was independent factor influencing the prognosis of locally advanced cervical cancer patients(P<0.01).Conclusion There are no differences for the recurrence rate and the short-term effica-cy of the three treatment methods.NST can improve the long-term efficacy of patients with local advanced cervical can-cer compared with RT or ST.
