名中医谢胜“以枢调枢”从肝肺论治溃疡性结肠炎理论探微

摘　　要：溃疡性结肠炎(UC)基本病机为脾虚湿盛,临床多从调脾胃论治,但部分患者疗效不尽如人意,且容易复发。谢胜教授认为上焦肺气郁闭,肺金肃而开宣不及,肝木郁而疏泄不及,肝肺枢机宣降失和,则脾土壅而升清不及,致脾胃枢机升降失衡,化生风火痰,表现在肠腑内生痈疡而发为UC。“脾虚湿盛”是UC的病机表象,其病机本质乃“肺气郁闭,肝肺枢机失和”。谢教授基于“以枢调枢”理论,通过调肝肺枢机以和脾胃升降枢机的方法治疗UC取得了良好疗效。The basic pathogenesis of ulcerative colitis(UC)is spleen deficiency and dampness excess,and most of them are treated by regulating spleen and stomach,but the curative effect of some patients is not satisfactory and is easy to relapse.Professor Xie Sheng believes that lung qi of Shangjiao is stagnant,lung gold is not enough to open and release,liver wood is stagnant and catharsis is not enough,liver and lung cardinals declare disharmony,then spleen soil is blocked but not clear enough,resulting in imbalance of spleen and stomach cardinal ascending and descending,resulting in wind,fire and phlegm,which is manifested in UC caused by carbuncle selection in intestinal viscera.Professor Xie Sheng believes that“pleen deficiency and dampness excess”is the pathogenesis of UC,and its pathogenesis essence is“stagnation of lung qi,liver and lung cardinal disharmony”.Based on the theory of“regulating Shu by Shu”,a good curative effect has been achieved in the treatment of UC by regulating liver and lung cardinal and lifting cardinal of spleen and stomach.
