摘　　要：目的:观察膀胱尿路上皮癌中吡咯啉-5-羧酸还原酶1(PYCR1)、增殖细胞核抗原(PCNA)和半胱氨酸天冬氨酸特异性蛋白酶-3(Caspase-3)的表达特征。方法:选择确诊为膀胱尿路上皮癌的患者63例作为观察组,选择经病理证实为正常膀胱黏膜的组织63例作为对照组,应用免疫组化法检测组织中PYCR1、PCNA和Caspase-3的表达,分析PYCR1、PCNA和Caspase-3在观察组不同临床病理特征中的表达差异及与术后生存时间的相关性。结果:PYCR1和PCNA在观察组中的阳性率均明显高于对照组,Caspase-3在观察组中的阳性率明显低于对照组(均P<0.05);观察组中PYCR1、PCNA和Caspase-3的表达与病变分级、浸润深度、脉管癌栓和是否伴鳞状分化密切相关(均P<0.05),而与性别、年龄、神经累犯无明显相关性(均P>0.05);PYCR1、PCNA和Caspase-3的表达均与术后生存时间有关;观察组中PYCR1与PCNA具有正相关性,PYCR1与Caspase-3具有负相关性,而PCNA与Caspase-3无明显相关性。结论:膀胱尿路上皮癌中PYCR1和PCNA高表达、Caspase-3低表达均参与病变的形成和进展,联合检测PYCR1、PCNA和Caspase-3的表达对判断预后有一定价值。PYCR1与增殖指标PCNA、凋亡指标Caspase-3均有一定关联性。Objective:To observe the expression characteristics of pyrrolin-5-carboxylate reductase 1(PYCR1),proliferating cell nuclear antigen(PCNA)and cysteine aspartate-specific protease-3(Caspase-3)in uroepithelial carcinoma of bladder.Methods:Sixty-three patients diagnosed with uroepithelial carcinoma of bladder were selected as the observation group,and 63 patients with normal bladder mucosa were selected as the control group.The expressions of PYCR1,PCNA and Caspase-3 were detected by immunohistochemistry.The expression differences of PYCR1,PCNA and Caspase-3 in different clinicopathological features of the observation group and their correlation with postoperative survival time were analyzed.Results:The positive rates of both PYCR1 and PCNA in the observation group were significantly higher than those in the control group,and the positive rate of Caspase-3 was significantly lower than that in the control group(all P<0.05).In the observation group,the expressions of PYCR1,PCNA and Caspase-3 were strongly correlated with lesion grade,depth of infiltration,vascular carcinoma thrombus and whether it was associated with squamous differentiation(all P<0.05),while there was no significant correlation with gender,age and nerve involvement(all P>0.05);the expressions of PYCR1,PCNA and Caspase-3 were associated with prognosis(all P<0.05);positive correlation was found between PYCR1 and PCNA,and negative correlation was found between PYCR1 and Caspase-3 in the observation group.Conclusion:High expressions of PYCR1 and PCNA,low expression of Caspase-3 are involved in the formation and progression of lesions in bladder uroepithelial carcinoma.The combined detection of PYCR1,PCNA and Caspase-3 expression is valuable for prognosis.There is a correlation between PYCR1 and PCNA,Caspase-3.
