摘　　要：目的:探究不同腹腔入路腹腔镜肾上腺肿瘤切除术在肾上腺肿瘤(Adrenal tumors,AT)患者治疗中的临床价值。方法:将本院2019年01至2022年01实施腹腔镜肾上腺肿瘤切除术的100例AT患者纳入研究对象,根据手术入路分为后腹腔入路组(即研究组51例)、腹腔入路组(参照组49例)。观察两组围术期指标、术前、术后1 d、3 d肾功能[肌酐(Serum creatinine,SCr)、尿素氮(Blood urea nitrogen,BUN)、尿红细胞(Urine red blood cell,URBC)]、并发症情况。结果:研究组手术时间、胃肠功能恢复时间、住院时间短于参照组,术中出血量少于参照组(P<0.05);术后1 d、3 d研究组血清BUN、SCr与参照组比较无显著差异(P>0.05);研究组术后1 d尿URBC高于参照组(P<0.05);两组并发症发生率(5.88%、18.37%)比较无显著差异(P>0.05)。结论:经后腹腔入路腹腔镜肾上腺肿瘤切除术治疗AT安全性较高,能优化手术路径,有助于胃肠功能恢复,缩短恢复进程。Objective:To explore the clinical value of laparoscopic adrenal tumor resection via retroperitoneal approach in the treatment of patients with adrenal tumors(AT).Methods:100 patients with AT who underwent laparoscopic adrenal tumour resection from 201901 to 202201 in our hospital were included in the study,and were divided into the posterior abdominal access group(51 patients in the study group)and the abdominal access group(49 patients in the reference group)according to the surgical access.The perioperative indicators,preoperative and 1 and 3 d postoperative renal function[creatinine(SCr),urea nitrogen(BUN),urine red blood cell(URBC)]and complications were observed in both groups.Results:The study group had shorter operative time,recovery time of gastrointestinal function and hospital stay than the reference group,and less intraoperative bleeding than the reference group(P<0.05);there was no significant difference in serum BUN and SCr between the study group and the reference group at 1 d and 3 d postoperatively(P>0.05);the urinary URBC was higher in the study group than the reference group at 1 d postoperatively(P<0.05);there was no significant difference in the incidence of complications between the two groups(5.88%,18.37%)(P>0.05).Conclusion:Laparoscopic adrenal tumor resection through retroperitoneal approach in the treatment of AT is safe,can optimize the surgical path,reduce the damage of immune function,help the recovery of gastrointestinal function,and shorten the recovery process.
