摘  要:目的探究血清尿酸(UA)、黄嘌呤氧化酶(XOD)表达水平与类风湿关节炎(RA)患者疾病活动度的关系。方法分析本院2019年3月至2021年1月接收的180例RA患者(RA组)的临床资料,根据28个关节的疾病活动度评分(DAS28)将患者分为缓解期组(86例)(DAS28评分<2.6分)和活动期组(94例)(DAS28评分≥2.6分),同时依据X线检查将患者分为骨损伤组(60例)和无骨损伤组(120例);对照组(176例)为健康体检者。血清UA水平的检测采用全自动生化分析仪,血清XOD水平的检测采用双抗体夹心酶联免疫吸附(ELISA)法;采用Pearson进行RA患者血清UA、XOD与疾病活动度的相关性分析;采用Logistic回归法分析UA、XOD对RA患者疾病活动度的影响。结果RA组血清UA、XOD表达水平明显高于对照组(P<0.05)。活动期组血清UA、XOD表达水平明显高于缓解期组(P<0.05)。活动期RA患者UA与XOD、UA与DAS28评分、XOD与DAS28评分均呈正相关(r=0.721、0.622、0.571,P均<0.05)。缓解期RA患者UA与XOD、UA与DAS28评分、XOD与DAS28评分均无显著相关性(r=0.246、0.283、0.271,P均>0.05)。UA、XOD是RA患者发展为活动期的独立危险因素(P<0.05)。结论RA患者血清UA、XOD表达水平较高,且活动期RA患者血清UA、XOD与DAS28评分相关性较好,二者可能成为评估RA疾病活动状态的敏感指标。Objective To explore the relationship between serum uric acid(UA)and xanthine oxidase(XOD)expression levels and disease activity in patients with rheumatoid arthritis(RA).Methods The clinical data of 180 RA patients(RA group)from March,2019 to January,2021 were analyzed.According to the disease activity score of 28 joints(DAS28),these patients were divided into(86 cases)remission group(DAS28 score<2.6 points)and(94 cases)active phase group(DAS28 score≥2.6 points).These patients were also divided into(60 cases)bone injury group and(120 cases)no bone injury group according to X-ray examination;During the same period,the control group(176 cases)were developed with healthy subjects.The detection of serum UA levels was carried out using a fully automated biochemical analyzer,while double antibody sandwich enzyme-linked immunosorbent(ELISA)method was used to detect the level of serum XOD;The correlation between serum UA,XOD and disease activity in RA patients analyzed by Pearson.Logistic regression was used to analyze the effects of UA and XOD on disease activity in RA patients.Results The expression levels of serum UA and XOD in the RA group were significantly higher than the control group(P<0.05).Compared with the remission group,the expression levels of serum UA and XOD in the active phase group were significantly higher(P<0.05).UA and XOD,UA and DAS28 scores,XOD and DAS28 scores of RA patients in the active stage were all positively correlated(r=0.721,0.622,0.571,all P<0.05).There was no significant correlation between UA and XOD,UA and DAS28 scores,XOD and DAS28 scores in RA patients in remission(r=0.246,0.283,0.271,all P>0.05).UA and XOD were independent risk factors for RA patients to develop into active phase(P<0.05).Conclusion The levels of UA and XOD in RA patients are relatively high,and there is a good correlation between serum UA and XOD in patients with active RA and DAS28 score,both of which could serve as sensitive indicators to assess the disease activity of RA.
