八段锦联合推拿治疗中风足内翻的效果及踝足运动学特征分析

摘　　要：目的:探讨八段锦联合推拿治疗中风足内翻的效果及对踝足运动学特征的影响。方法:选取我院70例中风后足内翻患者(2021年6月~2022年6月)并按随机数字表法分为研究组和对照组各35例,对照组给予常规康复训练及推拿治疗,研究组则在此基础上结合八段锦,持续治疗4周,评价治疗前后两组功能性步行量表(FAC)分级、临床痉挛指数(CSI)、Fugl-Meyer运动功能(FMA)评分及足跟动力学参数、步态参数的变化。结果:与对照组比较,研究组治疗后FMA评分、FAC分级评分更高,CSI评分更低(P<0.05);研究组治疗后步速、步长较对照组高,步态周期较对照组短(P<0.05);治疗后,研究组支撑相最大外翻、足跟触地时跖屈、内翻、内收及摆动相最大跖屈、最大内翻角度小于对照组,支撑相最大背屈角度大于对照组(P<0.05)。结论:八段锦联合推拿治疗中风足内翻能减轻肌肉痉挛,促进肌力恢复,改善步态及下肢运动功能,提高步行能力。Objective:To investigate the effect of Baduanjin combined with massage on apoplectic varus foot and its influence on foot and ankle kinematic characteristics.Methods:Seventy patients with apoplectic varus foot(from June 2021 to June 2022)in our hospital were selected and divided into study group(n=35)and control group(n=35)by random number table method.The control group was given routine rehabilitation training and massage,while study group combined with Baduanjin on the basis of the control group.After 4 weeks of treatment,the functional walking scale(FAC),clinical spasticity index(CSI),Fugl-Meyer Motor Function assessment(FMA)score,heel dynamic parameters and gait parameters were evaluated before and after treatment.Results:Compared with the conventional group,the FMA score and FAC grade were higher and CSI score was lower in the study group after treatment(P<0.05).After treatment,the step speed and step length of the study group were higher than those of the conventional group,and the gait cycle of the study group was shorter than that of the conventional group(P<0.05).After treatment,the maximum valgus Angle of support phase,plantar flexion,varus,adduction and swing phase,and the maximum dorsiflexion Angle of support phase in the study group were lower than those in the conventional group(P<0.05).Conclusion:Baduanjin combined with massage in the treatment of apoplectic varus foot can reduce muscle spasm,promote muscle strength recovery,improve gait and lower limb motor function,and improve walking ability.
