摘　　要：目的分析Ⅺ型胶原蛋白α1链(COL11A1)表达情况与乳腺癌患者病情程度、新辅助化疗效果及预后的关系。方法选取135例Ⅱ/Ⅲ期乳腺癌患者作为研究对象,所有患者均接受新辅助化疗。取患者新辅助化疗前的肿瘤组织标本,采用免疫组化法检测COL11A1、雌激素受体(ER)、孕激素受体、人类表皮生长因子受体2(HER2)、增殖细胞核抗原Ki-67的表达情况。分析COL11A1表达情况与患者分子分型、临床病理特征及预后的关系。结果COL11A1阳性患者的ER表达阳性率、HER2表达阳性率、Ki-67表达阳性率、TNMⅢ期比例、肿瘤直径>5 cm比例及腋窝淋巴结转移率均高于COL11A1阴性患者(均P<0.05)。COL11A1阳性患者的新辅助化疗总有效率、病理完全缓解率、总体无复发生存率均低于COL11A1阴性患者(均P<0.05)。结论COL11A1的表达情况与乳腺癌患者的病情程度、新辅助化疗效果及预后有关,COL11A1阳性患者的病情更重,新辅助化疗效果和预后更差。COL11A1有望成为乳腺癌患者新辅助化疗效果及预后的预测指标。Objective To analyze the relation of collagen typeⅪalpha 1 chain(COL11A1)expression with the severity,effect of neoadjuvant chemotherapy,and prognosis in patients with breast cancer.Methods A total of 135 patients with breast cancer in stageⅡ/Ⅲwere selected as the research subjects,and all patients received neoadjuvant chemotherapy.Samples of tumor tissues before neoadjuvant chemotherapy were obtained from patients,and the expressions of COL11A1,estrogen receptor(ER),progesterone receptor,human epidermal growth factor receptor 2(HER2),and proliferating cell nuclear antigen Ki-67 were detected by using the immunohistochemical method.The relation of COL11A1 expression with molecular typing,clinicopathologic features,and prognosis in patients was analyzed.Results The expression positive rates of ER,HER2,Ki-67,and proportions of TNM in stageⅢ,tumor diameter>5 cm,as well as metastasis rate of axillary lymph node in patients with COL11A1 positive were all higher than those in patients with COL11A1 negative(all P<0.05).The total effective rate and pathological complete response rate of neoadjuvant chemotherapy,and total recurrence-free survival rate of COL11A1 positive patients were all lower than those of COL11A1 negative patients(all P<0.05).Conclusion The expression of COL11A1 is related to the severity,effect of neoadjuvant chemotherapy,and prognosis in patients with breast cancer.Patients with COL11A1 positive exert more severe disease,and more poor neoadjuvant chemotherapy effect and prognosis.COL11A1 is expected to be a predictor of effect of neoadjuvant chemotherapy and prognosis in patients with breast cancer.
