摘　　要：目的探讨角膜接触镜不适感患者不适症状与泪液炎症因子水平的相关性。方法将69例佩戴角膜接触镜的近视患者作为研究对象,根据佩戴角膜接触镜后是否出现不适症状将患者分为症状组(n=37)和无症状组(n=32)。比较两组患者的泪液炎症因子[白细胞介素(IL)-2、IL-4、IL-6、IL-17A、肿瘤坏死因子α(TNF-α)、γ-干扰素]水平、泪液稳定性、泪液渗透压。分析症状组患者不适症状与泪液炎症因子水平、泪液稳定性的相关性。结果两组患者的泪液IL-2、IL-4、IL-6、IL-17A、TNF-α、γ-干扰素水平及角膜接触镜泪膜表面质量(CL-TFSQ)指数比较,差异均有统计学意义(均P<0.05)。相关性分析结果显示,症状组患者视觉角膜接触镜舒适度量表得分与泪液IL-2、IL-4、IL-6、IL-17A水平及CL-TFSQ指数呈正相关,与泪液γ-干扰素水平呈负相关,CL-TFSQ指数与泪液IL-2、IL-4、IL-17A、TNF-α水平呈负相关,与泪液γ-干扰素水平呈正相关(均P<0.05)。结论角膜接触镜不适感患者的不适症状,可能与其泪液炎症因子水平发生变化引起泪膜稳定性降低有关。Objective To investigate the correlation of uncomfortable symptoms with tear inflammatory factors levels in patients with contact lens discomfort.Methods A total of 69 myopic patients wearing contact lenses were taken as the research subjects.According to the presence of uncomfortable symptoms appeared after wearing contact lenses,patients were divided into symptom group(n=37)or asymptomatic group(n=32).The levels of tear inflammatory factors in terms of interleukin(IL)-2,IL-4,IL-6,IL-17A,tumor necrosis factorα(TNF-α),andγ-interferon,as well as tear stability,tear osmotic pressure were compared between patients of the two groups.The correlation of uncomfortable symptoms with the levels of tear inflammatory factors and tear stability of patients in the symptom group was analyzed.Results There were statistically significant differences in IL-2,IL-4,IL-6,IL-17A,TNF-α,γ-interferon levels,and contact lens tear film surface quality(CL-TFSQ)index between the two groups(all P<0.05).The results of correlation analysis revealed that visual contact lens comfort scale score of patients in the symptom group positively correlated with tear IL-2,IL-4,IL-6,IL-17A,TNF-αlevels,and CL-TFSQ index,and negatively correlated with tearγ-interferon level;in addition,CL-TFSQ index negatively correlated with tear IL-2,IL-4,IL-6,IL-17A,TNF-αlevels,and positively correlated with tearγ-interferon level(all P<0.05).Conclusion The uncomfortable symptoms of patients with contact lens discomfort may be related to the decline of tear film stability induced by the change of tear inflammatory factors levels.
