共同性外斜视患儿眼直肌后徙联合拮抗肌截除术后角膜曲率和眼球屈光度变化及其相关性分析

摘　　要：目的探究共同性外斜视患儿眼直肌后徙联合拮抗肌截除术后角膜曲率和眼球屈光度变化及其相关性。方法选择66例共同性外斜视患儿为研究对象,均接受眼直肌后徙联合拮抗肌截除术治疗。测定患儿手术前、后的患眼角膜曲率和眼球屈光度,并用Pearson相关性分析术后1个月不同区域各个方位的角膜曲率变化与眼球屈光度变化的相关性。结果患儿距离角膜顶点3、5 mm区域内鼻侧、颞侧方向的角膜曲率:术后1个月<术后1周<术前,上侧、下侧方向的角膜曲率:术后1个月>术后1周>术前(P<0.05)。患儿术后1周和术后1个月时的患眼眼球屈光度绝对值显著大于术前(P<0.05)。患儿患眼术后1个月时距离角膜顶点3、5、7 mm区域鼻侧、颞侧、上侧和下侧4个方向的角膜曲率与术前差值和术后1个月的眼球屈光度与术前差值无明显相关性(P>0.05)。结论眼直肌后徙联合拮抗肌截除术治疗共同性外斜视术后早期距角膜顶3、5 mm处垂直子午线4个方向的角膜曲率和眼球屈光度均会出现明显变化,但术后早期角膜曲率变化与眼球屈光度变化无明显相关性。Objective To explore the changes and correlation of corneal curvature and ocular diopter after rectus recession combined with antagonistic muscle resection in children with concomitant extropia.Methods A total of 66 children with concomitant exotropia were selected as the research objects.All the children were treated with rectus recession combined with antagonistic muscle resection.The corneal curvature and ocular diopter of the affected eye before and after surgery were measured,and the correlation between the changes of corneal curvature and the changes of ocular diopter in different regions at 1 month after surgery was analyzed by Pearson correlation coefficient method.Results The corneal curvature in the nasal and temporal directions in the area of 3 and 5 mm from the corneal vertex in children:1 month after operation<1 week after operation<preoperative,and the corneal curvature in the upper and lower directions:1 month after operation>1 week after operation>preoperative(P<0.05).The absolute value of the ocular diopter of the affected eye at 1 week and 1 month after operation was significantly higher than that before operation(P<0.05).There was no significant correlation between the difference of corneal curvature in the nasal,temporal,superior and inferior directions of the 3,5,7 mm region from the corneal vertex at 1 month after operation and that before operation and the difference between the ocular diopter and 1 month after operation and that before operation(P>0.05).Conclusion There are significant changes in corneal curvature and ocular diopter in the four directions perpendicular to the meridian at 3 and 5 mm from the corneal vertex in the early postoperative period after rectus recession combined with concomitant extropia for concomitant exotropia.However,there is no significant correlation between the changes of corneal curvature and ocular diopter in the early postoperative period.
