浙江省小儿窦性心率正常参考值的多中心研究

摘　　要：目的 探讨浙江省不同年龄段小儿窦性心率正常参考值。方法 选取2021年8月至2022年10月在浙江省7个有代表性地区的儿童医院、妇幼保健院或三甲综合性医院行心电图检查的6 321名健康小儿为研究对象,收集所有小儿静息状态下心率数据。根据小儿生长发育特点分为9个年龄段,分析得到浙江省不同年龄段小儿窦性心率正常参考值推荐标准。结果 在出生(足月儿)~1个月、1~12个月、7~16岁年龄段,7个地区小儿静息状态下心率比较差异均有统计学意义(均P<0.05);在其余6个年龄段,7个地区小儿静息状态下心率比较差异均无统计学意义(均P>0.05)。相邻两个年龄段小儿静息状态下心率比较,差异均有统计学意义(均P<0.05)。浙江省不同年龄段小儿窦性心率正常参考值推荐标准:出生(足月儿)~<1个月为100~170次/min,1~<12个月为100~155次/min,1~<2岁为90~140次/min,2~<3岁为85~135次/min,3~<4岁为80~130次/min,4~<5岁为75~120次/min,5~<6岁为70~115次/min,6~<7岁为65~110次/min,7~16岁为60~105次/min。结论 本研究明确了浙江省不同年龄段小儿窦性心率正常参考值,年龄分段较细致,符合临床需求,建议心电工作者根据本研究推荐标准评估小儿心率。Objective To investigate the normal reference value of sinus heart rate in children of different age groups in Zhejiang Province.Methods A total of 6321 healthy children undergoing electrocardiogram(ECG)examina-tions in seven representative pediatric hospitals,maternal and child health care hospitals or tertiary comprehensive hospitals in Zhejiang Province from August 2021 to October 2022 were included in the study,and the resting heart rate data of all children were collected.Children were divided into 9 age groups according to their growth and development characteristics,and the recommended standards for normal reference values of sinus heart rate in children of different age groups in Zhejiang Province were obtained.Results There were statistically significant differences in resting heart rate among children in the age groups of birth(full-term infant)to one month,one to twelve months and seven to sixteen years old in 7 regions(all P<0.05).However,in the other 6 age groups,there were no significant differences in resting heart rates among children in 7 regions(all P>0.05).There were statistically significant differences in resting heart rate between two adjacent age groups(all P<0.05).Recommended standard for normal reference values of sinus heart rate in children of different age groups in Zhejiang Province are as follows:100 to 170 beats/min from birth(full term)to 1 month;100 to 155 beats/min from 1 to 12 months;90 to 140 beats/min from 1 to 2 years old;85 to 135 beats/min from 2 to 3 years old;80 to 130 beats/min from 3 to 4 years old;75 to 120 beats/min from 4 to 5 years old;70 to 115 beats/min from 5 to<6 years old;65 to 110 beats/min from 6 to<7 years old;60 to 105 beats/min from 7 to 16 years old.Conclusion This study has provided normal reference values for sinus heart rate in children of different age groups in Zhejiang Province.It is recommended that ECG professionals use the recommended standards from this study to assess the heart rate in children.
