非小细胞肺癌组织中CK5/6的表达及与临床病理特征和预后的关系

摘　　要：目的 探讨非小细胞肺癌(NSCLC)组织中细胞角蛋白5/6(CK5/6)的表达及与临床病理特征和预后的相关性。方法 收集52例NSCLC患者的临床资料,检测其CK5/6表达情况,并比较不同临床病理特征者癌组织CK5/6表达差异。以1年为观察终点,比较不同癌组织CK5/6表达者总生存及无瘤生存情况。结果52例中22例癌组织CK5/6表达阳性,阳性率为42.31%。年龄≥65岁、有吸烟史、组织类型为鳞癌、低分化、淋巴结转移及TNM分期Ⅲ~Ⅳ期者癌组织CK5/6表达阳性率更高(均P <0.05)。肺鳞癌CK5/6表达阳性与表达阴性者1年总生存率、1年无瘤生存率及无瘤生存时间差异均无统计学意义(均P> 0.05)。肺腺癌CK5/6表达阳性者1年无瘤生存率、无瘤生存时间均低于表达阴性者(均P <0.05)。结论 CK5/6的表达不仅与NSCLC组织类型、恶性程度有关,还能辅助预测肺腺癌不良预后,指导临床诊疗。Objective To explore the expression of cytokeratin 5/6(CK5/6) in non-small cell lung cancer(NSCLC)tissues and its correlation with clinicopathological characteristics and prognosis.Methods The clinical data of 52patients with NSCLC were collected.The CK5/6 expression was detected,and the difference in CK5/6 expression was compared among different clinicopathological characteristics.With one year as the observation end-point,the overall survival and tumor-free survival were compared among patients with different CK5/6 expressions in cancer tissues.Results Among 52 cases,CK5/6 expression was positive in cancer tissues of 22 cases,with positive rate of42.31%.The positive rate of CK5/6 expression was higher in cancer tissues of patients with age≥65 years old,history of smoking,tissue type of squamous cell carcinoma,low differentiation,lymph node metastasis and TNM stage Ⅲ-Ⅳ(all P 0.05).Among patients with squamous cell lung carcinoma,there were no statistically significant differences in one year overall survival rate,one year tumor-free survival rate and tumor-free survival time between patients with positive and negative CK5/6 expression(all P 0.05).Among patients with lung adenocarcinoma,the one year tumor-free survival rate and tumor-free survival time of patients with positive CK5/6 expression were lower or shorter than those of patients with negative expression(all P 0.05).Conclusions The expression of CK5/6 is not only related to the tissue type and malignancy degree of NSCLC,but also can help predict the poor prognosis of lung adenocarcinoma and guide the clinical diagnosis and treatment.
