侵袭性葡萄胎的MRI表现与认识

摘　　要：目的探讨分析侵袭性葡萄胎(IM)的MRI表现,提高影像学表现的认识。方法选取23例病理明确诊断IM患者的临床及MRI资料进行回顾性分析。结果23例病灶均呈团块状,侵入肌层。T_(2)WI信号特点分三类:6例病灶内大量大小不等、边界清晰的囊状高信号;9例病灶内少量小囊或微囊状高信号;8例病灶呈稍高、等、低混杂信号。23例均可见肌层多发迂曲流空血管影。MRI动态增强:动脉早期病灶及临近肌层内扩张的血管显著强化;病灶实性部分逐渐强化;延时期病灶实性部分明显强化,囊变区、出血坏死区无强化;其中3例病灶内发现扩张的血窦。结论IM具有特征性MRI表现,深入理解和认识其影像特征有助于提高诊断水平。Objective This study analyzed the MRI manifestations of IM in order to further improve the cognition of imaging manifestations.Methods The clinical data and MRI of 23 cases of IM conformed by pathology were analyzed retrospectively.Results The lesion was a lump and invaded in myometrium in all 23 cases of IM.The features of lesions were divided into three kinds on T_(2)WI.6 cases showed lots of cysts with inequality of size,clear edges and hyper-signal on T_(2)WI.9 cases showed a few small or micro cysts with hyper-signal on T_(2)WI.8 cases showed the complex signal on T_(2)WI,containing slight hyper-signal,iso-signal and hypo-signal.The flow voids of vessels in myometrium were displayed clearly on T_(2)WI in all cases.During the early phases of dynamic contrast enhancement,the dilated and enhanced blood vessels in lesions and myometrium were displayed clearly.The solid components were gradually enhanced and significantly enhanced in the delayed phase,but hemorrhagic and necrotic areas and cystic components had no enhancement.The dilated blood sinuses were found in 3 cases.Conclusion MRI findings of IM have characteristics,the further understood and cognized can help to improve the level of diagnosis.
