面部恶性纤维组织细胞瘤1例报告并文献复习

摘　　要：目的 探讨面部恶性纤维组织细胞瘤的流行病学特点、临床表现、病理及免疫组化特点、治疗以及治疗进展。方法 回顾性分析1例面部恶性纤维组织细胞瘤的临床资料,结合相关文献分析讨论该病的流行病学特点、临床表现、病理及免疫组化特点、治疗以及治疗进展。结果 患者右侧面部肿物1年,CT检查发现右侧颧弓旁面颊部可见1.0x2.0cm软组织肿块影,周围骨质未见破坏,未见明显肿大淋巴结,MRI检查见右侧面颊表皮可见椭圆形等T1、稍长T2信号影,边缘光整,其内可见条状长T1、短T2信号影,临近颧骨未见骨质破坏,行面部肿物切除术、自体皮瓣移植术,病理及免疫组化结果为面部恶性纤维组织细胞瘤。结论 面部恶性纤维组织细胞瘤为少见的恶性肿瘤,易误诊、误治;病史、CT及MRI检查等是诊断面部恶性纤维组织细胞瘤的重要手段,确诊依靠病理积免疫组化检查;治疗上推荐彻底除术联合术后辅助放疗;如今对恶性纤维组织细胞瘤的分子研究显示,多种因子可对其发生、运动、侵袭、转移及预后提供最新线索。Objective To investigate the epidemiological characteristics,clinical manifestations,pathological and immunohistochemical characteristics,treatment and treatment progress of facial malignant fibrous histiocytoma.Methods The clinical data of 1 case of malignant fibrous histiocytoma were analyzed retrospectively.The epidemiological characteristics,clinical manifestation,pathology and immunohistochemical characteristics,treatment and treatment progress of the disease were discussed and analyzed.Result Patient on the right side of the surface of the tumor 1 year,CT examination found the right side of the zygomatic arch cheek can be seen 1.0x2.0cm soft tissue mass shadow,no damage around the bone,no obvious lymph nodes,MRI examination see the right cheek skin visible Oval and other T1,slightly longer T2 signal shadow,the edge of light,which can see the strip long T1,short T2 signal shadow,near the cheekbones no bone destruction,line facial resection,autologous flap transplantation,Pathological and immunohistochemical results for facial malignant fibrous histiocytoma.Conclusion The history of malignant fibrous histiocytoma is a rare malignant tumor,which is easy to misdiagnosis and misdiagnosis.The history,CT and MRI are the important means to diagnose facial malignant fibrous histiocytoma.The diagnosis is based on immunohistochemical examination.In addition to postoperative adjuvant radiotherapy;now on the malignant fibrous histiocytoma molecular studies have shown that a variety of factors for its occurrence,movement,invasion,metastasis and prognosis to provide the latest clues.
