摘  要:近年来恶性肿瘤成为影响人类健康的主要原因,随着中国人均寿命的逐渐延长,人们罹患恶性肿瘤的概率也在逐渐增大。盆腔肿瘤的治疗方式基本以根治性手术切除为主,而在盆腔转移瘤上缺少较为有效的治疗手段,且肿瘤转移至盆腔淋巴结及软组织的情况也较为常见,对于此类患者一线药物治疗失败后更换二线药物控制原发肿瘤及盆腔转移瘤的疗效多不甚理想,所以患者的生活质量通常也较为低下。随着碘-125(^(125)I)粒子植入术的应用发展,越来越多的患者在晚期姑息减症治疗中应用此手术明显提高了肿瘤局部控制率和生活质量。对于盆腔肿瘤粒子植入的深度、方向、排布均有着严格的要求,这也意味着对于手术医生来说提出了更高的要求。近年来3D打印技术的发展,3D打印模板下的粒子植入配合术前TPS计划及术后剂量验证,此类手术的难度明显下降且有效率明显提升。本文旨在总结近年来粒子植入在盆腔肿瘤及转移瘤的发展,展望3D打印模板下的粒子植入术在未来的应用。In recent years,malignant tumors have become the main cause affecting human health.With the gradual extension of average life expectancy in China,the probability of malignant tumors is also increasing.In the treatment of pelvic tumors,radical surgical resection is the main method,while there is a lack of effective treatment for pelvic metastatic tumors,and it is also common for tumors to metastasize to pelvic lymph nodes and soft tissues.For such patients,replacing second-line drugs to control primary tumors and pelvic metastases after first-line drug treatment fails is often not satisfactory.So the quality of life is often lower.With the development of iodine-125(^(125)I)particle implantation,more and more patients are applying this surgery in the treatment of advanced palliative reduction,which significantly improves the local tumor control rate and quality of life.There are strict requirements for the depth,direction and arrangement of pelvic tumor particle implantation,which also means that higher requirements are put forward for surgeons.With the development of 3D printing technology in recent years,particle implantation under 3D printing templates combined with preoperative TPS planning and postoperative dose verification,the difficulty of such operations has been significantly reduced and the efficiency has been significantly improved.This paper aims to summarize the development of particle implantation in pelvic tumors and metastases in recent years,and look forward to the future application of particle implantation under 3D printing templates.
