物理性神经阻滞联合镜像疗法在脑卒中偏瘫性肩痛康复治疗中的应用

摘　　要：目的探讨物理性神经阻滞联合镜像疗法在脑卒中偏瘫性肩痛康复治疗中的应用效果。方法选择2022年1月至10月九江市第一人民医院收治的76例脑卒中偏瘫性肩痛患者作为研究对象,采用随机数字表法将其分为对照组(38例)与研究组(38例)。对照组采用常规康复训练,研究组采用物理性神经阻滞联合镜像疗法治疗。比较两组的临床疗效、患侧关节活动度、视觉模拟评分法(VAS)、Fugl-Meyer感觉运动恢复量表(FMA)评分及Barthel指数(BI)评分。结果研究组总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗前肩关节活动度、VAS、FMA、BI量表评分比较,差异无统计学意义(P>0.05)。两组治疗后肩关节活动度、FMA、BI评分均高于治疗前,VAS评分均低于治疗前,且研究组治疗后肩关节活动度、FMA、BI评分均高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。结论脑卒中偏瘫性肩痛康复治疗中应用物理性神经阻滞联合镜像疗法效果较好,利于减轻疼痛程度,改善肩关节活动度,提升运动、日常生活能力,值得临床广泛应用。Objective To explore the application effect of physical nerve block combined with mirror therapy in the rehabil-itation treatment of shoulder pain in stroke patients with hemiplegia.Methods A total of 76 patients with hemiplegic shoul-der pain after stroke treated in Jiujiang First People's Hospital from January to October 2022 were selected as the study ob-jects,and were divided into control group(38 cases)and study group(38 cases)by random number table method.Patients in the control group received routine rehabilitation training,while in the study group,participants received physical nerve block combined with mirror therapy.The clinical efficacy,affected joint range of motion,visual analog scale(VAS),Fugl-Meyer assessment(FMA)score,and Barthel index(BI)score between the two groups were compared.Results The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in range of motion of shoulder joint,VAS,FMA,or BI scores between the two groups before treatment(P>0.05).After treatment,the range of motion of shoulder joint,FMA,and BI scores of the two groups were higher than before treatment,and the VAS scores were both lower than before treatment.Moreover,the range of motion of shoulder joint,FMA,and BI scores of the study group were all higher than those of the control group,and the VAS score was lower than that of the control group,with statistically significant differences(P<0.05).Conclusion The applica-tion of physical nerve block combined with mirror therapy in the rehabilitation treatment of shoulder pain in stroke patients with hemiplegia obtains a favorable effect,which is beneficial for reducing the degree of pain,improving the range of motion of shoulder joint,enhancing motor and daily life abilities,and is worthy of widespread clinical application.
