摘 要:目的观察前列腺癌患者骨转移的发生与血浆纤维蛋白原表达水平的关系,探讨血浆纤维蛋白原诊断前列腺癌骨转移的临床价值。方法选取2019年1月至2022年1月就诊于石河子大学医学院第一附属医院的60例前列腺癌患者作为研究对象,收集其病历资料,统计发生骨转移的前列腺癌病例数。比较骨转移、非骨转移患者的年龄、临床分期、前列腺特异抗原(PSA)、白细胞计数、血浆纤维蛋白原水平、血肌酐、Gleason评分、体重指数(BMI)以及有无家族史、高血压、糖尿病、吸烟史、饮酒史。多因素logistic分析前列腺癌患者发生骨转移的高危因素。参照病理检查结果,计算血浆纤维蛋白原诊断前列腺癌骨转移的准确性、特异度和灵敏度。结果60例前列腺癌患者经病理检查证实16例骨转移,占26.67%;骨转移患者与非骨转移患者的临床分期中T3~T4期比例、PSA、血浆纤维蛋白原、Gleason评分比较,差异有统计学意义(P<0.05)。多因素logistic分析显示,临床分期T3~T4期(β=1.123,OR=2.078,95%CI=1.505~4.387)、Gleason评分升高(β=0.776,OR=2.001,95%CI=1.909~2.168)、血浆纤维蛋白原高表达(β=1.786,OR=3.376,95%CI=2.543~11.908)是前列腺癌患者发生骨转移的高危因素(P<0.05)。参照病理检查结果,血浆纤维蛋白原诊断前列腺癌骨转移的准确性为93.33%、特异度为97.73%、灵敏度为81.25%。结论血浆纤维蛋白原表达水平越高,前列腺癌患者发生骨转移的风险就越大,可以作为前列腺癌骨转移的辅助诊断指标。Objective To observe the relationship between the occurrence of bone metastasis in prostate cancer patients and to explore the clinical value of plasma fibrinogen in the diagnosis of bone metastasis of prostate cancer.Methods A total of 60 prostate cancer patients who visited the First Affiliated Hospital of Shihezi University Medical College from January 2019 to January 2022 were selected as research subjects,medical record data collected,the number of prostate cancer cases with bone metastasis was counted.Age,clinical stage,prostate-specific antigen(PSA),white blood cell count,plasma fibrinogen level,serum creatinine,Gleason score,body mass index(BMI),and have or not have family history,hypertension,diabetes,smoking history,drinking history of patients with bone metastasis and non bone metastasis were compared.logistic of high-risk factors for bone metastasis in prostate cancer patients were analyzed.The accuracy,specificity and sensitivity of plasma fibrinogen in diagnosing bone metastasis of prostate cancer were calculated based on pathological examination results.Results Sixteen out of 60 prostate cancer patients were confirmed to have bone metastasis through pathological examination,accounting for 26.67%.The clinical staging ratios of T3 to T4,PSA,plasma fibrinogen and Gleason scores were compared between patients with bone metastasis and non bone metastasis,the differences were statistically significant(P<0.05).According to logistic analysis,the clinical staging from T3 to T4(β=1.123,OR=2.078,95%CI=1.505-4.387),Gleason score increased(β=0.776,OR=2.001,95%CI=1.909-2.168),high expression of plasma fibrinogen(β=1.786,OR=3.376,95%CI=2.543-11.908)were high-risk factors for bone metastasis in prostate cancer patients(P<0.05).According to the pathological examination results,the accuracy,specificity and sensitivity of plasma fibrinogen in diagnosing bone metastasis of prostate cancer were 93.33%,97.73%and 81.25%,respectively.Conclusion The higher the expression level of plasma fibrinogen,the greater the risk o
