高血压患者感知服务质量及其行为依从性的关系研究——以医生信任为中介

摘　　要：目的:调查杭州市高血压患者感知服务质量、医生信任及高血压行为依从性现状以及三者之间的路径关系和作用机制。方法:运用慢性病管理评价量表(PACIC)、维克森林医师信任量表中文修订版(C-WFPTS)和高血压依从性量表(CHPS)为研究工具调查356名高血压患者,利用AMOS24.0构建结构方程模型并进行路径分析。结果:PACIC、C-WFPTS中仁爱维度和技术能力维度以及CHPS的条目均分分别为(3.05±0.66)、(3.85±0.65)、(3.99±0.57)和(3.35±0.40)分;仁爱信任和技术能力信任均在感知服务质量和高血压行为依从性之间起完全中介作用。结论:基层医疗卫生机构应注重提高慢性病管理质量以及提高医务人员的技术能力和服务态度水平,从而提高患者高血压行为依从性。Objective To investigate the status of perceived service quality,physician trust and behavioral compliance of hyertensive patients,as well as the relationship and operating mechanism among them.Methods A total of 356 hypertensive patients were investigated using Patients Assessment Chronic lIness Care scale(PACIC),Chinese version of W ake Forest Physician Trust Scale(C-WFPTS)and Compliance of Hypertensive Patients''scale(CHPS).The software of AMOS 24.0 was used to construct a.structural equation model and conduct path analysis.Results The average score of the total scale of PACIC,dimensions of benevo-lence and competence of C-WFPTS,and the total scale of CHPS was 3.05±0.66,3.85±0.65,3.99±0.57 and 3.35±0.40,respectively.Both benevolence and competence played a full mediating role in the relationship between perceived service quality and hypertensive behavioral compliance.Conclusion Primary medical and health institutions should focus on improving the quality of chronic disease management and improving the technical ability and service attitude of medical stafs,so as to improve the hyper-tensive behavioral compliance.
