摘 要:目的:探究超声引导下细针穿刺细胞学检查(US-FNAC)对甲状腺良恶性肿瘤的诊断价值,以期为临床甲状腺良恶性肿瘤鉴别诊断提供参考依据。方法:选取2019年1月—2021年1月新乡市中心医院收治的98例甲状腺结节患者作为研究对象,分别对其进行常规超声初筛、US-FNAC及病理学检查。统计患者病理细胞学组织活检诊断结果,比较超声常规初筛与US-FNAC对甲状腺肿瘤恶性检出率,以病理细胞学组织活检结果为“金标准”,比较两种方法对甲状腺良恶性肿瘤的诊断价值。结果:常规超声初筛对甲状腺恶性肿瘤检出率低于US-FNAC,差异有统计学意义(P<0.05);以病理细胞学组织活检结果为“金标准”,US-FNAC诊断敏感度、特异性及准确率高于超声常规初筛,差异有统计学意义(t=7.207、17.848、10.714,P<0.05);US-FNAC过程中,有1例患者出现穿刺部位肿痛。结论:在甲状腺良恶性肿瘤诊断过程中,使用US-FNAC敏感度、特异性及准确率均较高,且具有一定安全性,可作为诊断该疾病的辅助手段。Objective:To investigate the diagnostic value of ultrasound-guided fine-needle aspiration cytology(US-FNAC)on benign and malignant thyroid tumors,in order to provide a reference basis for the differential diagnosis of benign and malignant thyroid tumors in clinical practice.Methods:98 patients with thyroid nodules admitted to the hospital from January 2019 to January 2021 were selected for the study and were subjected to routine ultrasound primary screening,US-FNAC and pathological examination.Compared the detection rate of malignant thyroid tumor between conventional ultrasound screening and US-FNAC.The diagnostic results of the patients’pathological cytology tissue biopsies were counted,and the malignancy detection rate of thyroid tumors was compared between conventional primary screening by ultrasound and US-FNAC.The diagnostic value of the two methods for the diagnosis of benign and malignant thyroid tumors was compared using pathological cytology biopsy as the“gold standard”.Results:The detection rate of conventional ultrasound primary screening for thyroid malignancy was lower than that of US-FNAC,and the difference was statistically significant(P<0.05).With pathological cytological tissue biopsy as the“gold standard”,the sensitivity,specificity and accuracy of US-FNAC diagnosis were higher than those of conventional ultrasound primary screening,with statistically significant differences(t=7.207,17.848,10.714,P<0.05).During US-FNAC,1 patient developed swelling and pain at the puncture site.Conclusion:In the diagnosis of benign and malignant thyroid tumors,the use of US-FNAC has high sensitivity,specificity and accuracy,and has a certain degree of safety,and can be used as an adjunct to the diagnosis of this disease.
