维A酸乳膏联合得宝松局封治疗瘢痕疙瘩疗效分析

摘　　要：目的了解外用维A酸乳膏联合复方倍他米松注射液(商品名:得宝松)局部封闭(局封)治疗瘢痕疙瘩的临床效果及安全性。方法40例瘢痕疙瘩患者,随机分为对照组和治疗组,各20例。对照组采用得宝松注射液皮损内注射治疗,治疗组采用得宝松注射液皮损内注射联合维A酸乳膏治疗。比较两组治疗9、18、27周后的疗效以及27周后不良反应发生情况。结果两组患者治疗9、18周后的治疗总有效率比较,差异均无统计学意义(P>0.05)。治疗27周后,治疗组治疗总有效率95%高于对照组的70%,差异具有统计学意义(P<0.05)。两组患者行皮损内注射时均出现不同程度的疼痛;治疗27周后,治疗组出现1例红肿,1例皮肤萎缩,2例毛细血管扩张,不良反应发生率为20%;对照组未出现红肿,出现2例皮肤萎缩,1例毛细血管扩张,不良反应发生率为15%。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论维A酸乳膏联合得宝松局部封闭治疗瘢痕疙瘩明显提高疗效,且未增加不良反应。Objective To understand the clinical effect and safety of topical tretinoin cream combined with compound betamethasone injection(trade name:Diprospan)local block in the treatment of keloid.Methods A total of 40 patients with keloid were randomly divided into control group and treatment group,with 20 cases in each group.The control group was treated with intralesional injection of Diprospan injection,and the treatment group was treated with intralesional injection of Diprospan injection and tretinoin cream.The efficacy after 9,18 and 27 weeks,and the occurrence of adverse reactions after 27 weeks were compared between the two groups.Results There was no statistically significant difference in the total effective rate between the two groups after 9 and 18 weeks of treatment(P>0.05).After 27 weeks of treatment,the total effective rate of 95%in the treatment group was higher than that of 70%in the control group,and the difference was statistically significant(P<0.05).In both groups,patients experienced varying degrees of pain during intradermal injection;after 27 weeks of treatment,1 case of erythema,1 case of skin atrophy and 2 cases of capillary dilatation occurred in the treatment group,and the incidence of total adverse reactions was 20%;in the control group,there was no redness and swelling,2 cases of skin atrophy and 1 case of capillary dilation,and the incidence of total adverse reactions was 15%.There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Tretinoin cream combined with Diprospan local block in the treatment of keloids can significantly improve the efficacy and did not increase adverse reactions.
