高建忠自拟解郁方治疗郁证临证经验

摘　　要：郁证是一类由情志忧郁、气机郁滞引起,且以情绪低落为特征的疾病。山西省中西医结合医院高建忠主任认为,郁证多为气郁,属于内伤。他从内伤角度出发,治疗上以通为原则,着眼于恢复脏腑功能与脏腑之间的关系,自拟解郁方治疗,使患者气郁无从产生,心神得安。他还观察到郁证患者常因脾气郁滞而出现痰气内阻的症状,因此运用门氏联合方组的思想,将解郁方与温胆汤联合治疗,临床疗效良好。文章结合两则案例,患者甲为气郁型郁证,治以自拟解郁方调畅肝脾气机,安心养神;患者乙为气滞痰阻型郁证,治以解郁方调畅气机,重镇安神,和温胆汤理气化痰。两例均疗效显著。文章介绍高建忠主任治疗郁证经验,以期为临床参考。Depression syndrome is a disease characterized by depression,which is caused by emotional depression and stagnation of Qi(气).GAO Jianzhong,a director of the Integrated Traditional Chinese and Western Medicine Hospital,believes that depression syndrome is mostly Qi depression,which belongs to internal injury.From the perspective of internal injury,he took dredging as the principle,focused on restoring the functions of the Zang-fu(脏腑)organs and the relationship between the zangfu organs,and prepared self-made Jieyu prescription(解郁方)for treatment,which made the patients unable to obtain Qi depression and relieved their mind.He also observed that patients with depression syndrome often had symptoms of phlegm-Qi obstruction due to spleen-Qi stagnation.Therefore,based on the idea of Men’s combined prescription therapy,the combination of Jieyu psescription and the Wendan decoction(温胆汤)had a definite clinical effect.The article combines two cases,patient A is Qi stasis type of depression,treatment with self-made Jieyu prescription to adjust the liver and temper machine,peace of mind and nourishment.The patient B is a stagnation of phlegm resistance type of depression,and the treatment is to relieve depression and adjust the Qi machine,focus on calming the spirit,and warm gallbladder soup to dissolve phlegm.Both cases had significant curative effects.This article introduces the Director Gao Jianzhong’s experience in the treatment of depression,in order to serve as a clinical reference.
